Confused whether acute or chronic

I had got hep-b vaccination when i was 13yrs old.Now when i got a regular medical check up done my sgpt/sgot was very high(SGOT-188,SGPT-479).
On further check i my results were as follows:
HBV DNA 1284000 iu/ml
Prothrombin Time 13.4sec
INR 1.01
Anti Hav Igm            Negative
Anti Hev IGM            Negative
Anti body to Heptatis C:Negative
HBeAG;                    Negative
Anti HBcAG-IgM    Negative
Anti HBc (Total)         Positive
Ultrasound was normal and liver doppler test was normal

Post that i was asked to tak entacavir 0.5mg daily and now  after 3.5 months of 1st diagnosis my viral count has come down to 625iu/ml and SGPT/SGOT are also normal.

The doubt that i have is

1   Am i suffering from chronic or acute Hep-b
2   How did i get it since i was vaccinated as a child?
3  Can this be cured
4 Can i consume beer once in a while

sorry for troubling you but one quick one.i was under the impression that antivirals had no affect on HBSAG count,then how come my hbsag quantitative went down from 9383 to 5865 iu/ml in 2 weeks.

if hbcab igm is negative it is chronic, if hbcab igg is pos it is acute

Hey got my test results again today ,HBV DNA has come down to 95 iu/ml and HBSag quantitative has become 5865 iu/ml.

1 HBsag has reduced from 9383 to 5865 iu/ml in 2weeks--is it because of  entacavir?does the count fluctuate like this? can it be a case of acute hapatitis.will the count increase??

how long before i can get my wives hbsab level checked after the booster dosage

no that s immune system cytokines.liposomals, vit d3 and fish oil high dha/epa at least 3g are needed to balance.check thyroid too

2 Does chronis HEP-B/baraclude have an impact on memory.i seem to not remember things that well.

1 if the hbv dna becomes undetectable will the hbsag number also come  down?

no they are two different things, it may come down but it is not correlated to hbvdna, you can have hbvdna und or detectable and hbsag going down


2 is there a medicine to reduce hbsag count

no the only way is using tenofovir for 5 years (50% get hbsag below 1000iu/ml by 5 years) and when hbsag is so low add on peginterferon.doing this most will lose hbsag

3 i had heard there were some clinical trials with birinapant+entecavir which removed hbsag completly

these are trials and i am not sure if it removed hbsag, link the study

4 what does the count depend on?why is it that some have high and some low in chronic HBV also

hbsag depends on number of infected cells and your immune system being able to kill infected cells

What should i do next.Whats the way forward?

keep using the antiviral, this is not acute hbv hbsag is too high

entecavir is not the best choice, tenofovir is more potent on hbsag when hbvdna is fully undetectable for few months you may choose to switch to tenofovir

well got my result today.
Hbsag quantitative is 9383.25 iu/ml, hbv dna 215 iu/ml, LFT normal with bilirubin direct at 0.24 and total biluribin at 1.22.
I am now on entacavir-bara clude for the past 3months and original hbvdna count was 1284000.
What should i do next.Whats the way forward?

little effect on hbsag, very few patient get reduction while 50% get levels below 1000iu/ml by 5 years of tdf (not etv)

Got my blood test for HBSAG quantitative awaiting results.Does baraclude-entracavir cure HBSAG damaged cells.As in does it help in reducing the count.

no complitely different test with nothing to do in common, hbvdna is replication and hbsag reflects infected cells.you can have undetectable replication but a lot of infected cells

tdf is more potent especially on hbsag although all antivirals have little effect on it

you got it in childhood probably

it was not high, you can keep also etv, just check that hbvdna gets und by 6-8month otherwise best tdf

you are missing the most importan t test, hbsag quantitative in iu/ml, this will tell you if you are getting rid of infected cells, have this test as soon as possible because you need a baseline value and then follow every 6-12motnhs

chronic before you took vaccine, probably at birth or when very young

2   How did i get it since i was vaccinated as a child?

if your mother has hbv you must take vaccine at birth, if mother negative you need it anyway when very very young...all adults clear so taking vaccine at 13yo is not that useful, too late

3  Can this be cured

hbsag lowers after 5 years of tenofovir, the most potent hbv antiviral.entecavir was not the best choice unless you have damaged kidneys

4 Can i consume beer once in a while

Chances are, you were chronic since birth. When you parents vaccinated you at 13, you probably had a tiny bit in you but at the time more tests were not available and your parents just did the standard thing of vaccination. Most chronic cases are from birth and come from the mother. If your mother can test for Hep B, then you can confirm that this is how you got it. There is also a chance you got it within the first 5 years of your life and never cleared because immunity is low. Your viral DNA suggests that it's chronic, because chronic is usually a lower number like this.

Unfortunately, no cure. 95% of acute cases in adult clear it naturally but again this does not sound like an acute case.

Stay away from beer (though my doctor told me drinking on occasion was ok.. and when he means by occasion, have maybe one every few months AT an occasion like a wedding is ok and even still, be careful!)
