DNA and ALT results - Interferon

Good day to all the member lists!

I just got the results of my recent Lab test. Below is my baseline.

FEB 2008
HBV DNA  377 IU/mL
ALT 118

TREATMENT OF INTERFERON FEB 08 TO SEPT 08 (with reference values above)

AUG 19, 2008
HBV DNA < 6 IU/mL
ALT 544

QUESTIONS:

1. What does it mean with the latest lab results?
2. Why the ALT is rocket high? Is this serious?
3. Do I have a chnace in clearing the virus?
4. Should I continue to Interferons?

For chronic active hepatitis B ALT levels may remain constant or decrease over the course of treatment however if ALT flares to >= 2 time base level then treatment with Intron-A should be continued unless signs or symptoms of liver failure are observed. If ALT flare occurs blood tests should be carried out at 2 weekly intervals. In chronic HB this may be a good sign because in some people this flare is coincident with clearance of viral DNAp.

After 1 months of treatment if markers for viral replication or HBeAg do not decrease the dose may be escalated according to tolerance of IF. If after 3 to 4 months no improvement has been seen then discontinuation of therapy should be considered. If some improvement is seen, they may wish to extend the IF course to 6 months.

After treatment with interferon they may wish to take another biopsy to assess the effectiveness of the treatment.

My current treatment is standard Interferon 5 MIU 3x a week

HbeAg - Negative
Anti-Hbe - Reactive

A:  The treatment is fighting the virus.

2. Why the ALT is rocket high? Is this serious?

A:  It is expected.  Once job done, it becomes normal.

3. Do I have a chnace in clearing the virus?

A:  Yes.

4. Should I continue to Interferons?

A:  You should complete the treatment which is working as indicated by the great drop of VL.

Hi.. the great point is the viral load decrease.. it’s almost undetectable and this is due to the therapy with IFN, I’d say.

Concerning Alt values  be advise they may rise up during tx and get normal with the time.

What is your Hbeag status? Are you eag positive or eag negative?

On which drugs are you? Pegasys? What about side effects you experienced?..

The Doc will suggest you about the tx elongation time…
