HEPATITIS B COMMUNITY
Eleveted SGOT/AST

Eleveted SGOT/AST

My mother is 52 years old  now. She has done a liver function test two days ago, and the result is as follows:

SGOT/AST    40IU/I
SGPT/ALT    58IU/I

Alpha-fetoprotein   2.1 ng/ml

Both SGOT and SGPT levels are elevated while alpha-fetoprptein is at normal level. What does this mean?

My mother is a hepatitis B carrier. She never drinks alcohol. Recently she feels very tired easily. She also feels pain in her right abdomen.I am very worried about her.

1) Is the chance of my mother having hepatitis great?
2) Can she do anything about it at this stage?
3) What are the complications if she really has hepatitis?
4) Will it developed into liver cancer?
5) What is the prognosis of having hepatitis?

I have found lab results vary with each clinic. Currently mine are as follows - Kaiser and UCSF:
AST (SGOT) 10-40
ALT (SGPT) <36
AFP (non-maternal) <15

While I was at UCLA Med Center their score ratings were:
AST (SGOT) 15-50
ALT (SGPT) 5-50
AFP (non-maternal)35-110

EVERY lab is different. You should ask for you mother's lab results and then look at the low and high water marks. You didn't state what type of AFP lab your mom had. AFP (non-maternal) is a marker for cancer cells but is not always correct. Its an indicator so other markers may disrupt its actual reading. But its the only AFP one should get to help rule out liver cancer. If it rises dramically over several months than it becomes a true indicator of liver cancer.
I suggest strongly that you rule out any growth factors on the liver by having a CT Scan with CONTRAST performed, or an MRI w/contrast ASAP. You must have the contrast which is a dye that highlites any hard material on the liver - tumors, cirrohsis. The first scan I had was w/o the contrast cuz the dummy Radiologist could not locate a proper vein. This scan showed nothing and he gave me a clean bill of health. My doc was very unhappy and ordered another scan w/contrast. This scan showed a 1.2 cm lesion next to my portal vein. This was in late Oct 03. By mid January the tumor had grown to 2.1 cms. The only cure was a transplant which I had on 03/19/04. I wouldn't be writing this if it wasn't for that last scan.
My surgical team believes my HCV came from a transfusion in 1954. I believe from an infected dental (novacain shots) or medical(tetanus, etc) needles prior to 1990. Get a Hepatitis panel test which will screen the testees blood for all the known hep virus'. I hope this helps.
