HBV DNA Spike, treatment

Am I going through a flare for my hbv dna?

5/19 hbv dna , 220 iu/ml - alt 50 , ast 34
6/24 hbv dna , 5190 iu/,ml - alt 56 ast 26

i am hbeag negative hbeab positive. infected at birth 37 years old. prev years alt 24 - 40  and hbv dna undetectable. in USA so no hbsag test.

platelets 171
afp 2.1

can someone please give me some advice. dont see gi doc until 7/22. thanks

The rise in your hbvdna is significant, but whether it is a spike or a consistent elevation is not easy to decide. In my opinion, you should repeat the viral load test in another one or two month time and check your ALT too. You may get a better picture then. If you should need treatment, and it is just a "if" at the moment, you may like to try Entecavir as you may have some prior kidney issue. A fibroscan, being a very simple (10 minutes) and non-invasive procedure, is always useful. Discuss with your doctor.

i read many of your post that you have made and it is a blessing. I started reading your post in the last 3 weeks and started vitd at 5000 iu. I told my gi doc about checking vitd level and he asked me why. I was a little concern, but in the process of changing to a hep specialist. I did a US but still waiting for the results.

the most important test for you now is fibroscan, do you regularly monitor it once a year?

i have never done a fibroscan and waiting for the US and after will proceed with the fibroscan. I have a bmi of 29 and working very hard to lower that. Do you think that will affect the fibroscan results?

I have a family history of hep b , no one has done genotype. Do you think I should do the genotype test and if genotype C should I start nuc right away. I also have some minor kidney issues with egfr at 72. I am worried the nucs will damage the kidneys even more. I read some great research on telbvudine + tenf combo. If I were to start nuc should I take telb and then wait for resistance then add tenf.

do you recommend any other supplement besides vitd 3 in USA. I told my doc about hepatech but they seem clueless.

the most important test for you now is fibroscan, do you regularly monitor it once a year?

small differences in hbvdna of 1log are not meaningful, i d check fibroscan and ultrasound and then choose if to start an antiviral for a future pegintf add/new drugs add on in 5 to 10 years or stay with no therapy

to note that any future therapy still need an antiviral and especially pegintf, without immune modulation no drug is showing any effect

also monitor your vitd and intact pth

Never had any nucs or treatments . I think I might be immune escaping from inactive phase but can it be that fast
