Can you explain this report and what type of precaution i require
Liver biopsy report?
Section of liver core show effacement of architecture. Severe portal inflammation is seen composed of lymphocytes, neutrophils, eosinophils and few plasma cells. Marked interface hepatitis is seen. Mild bile ductular proliferation are present. Hepatic lobules show lobular inflammation and hepatic rosettes along with collapse. Bridging necrosis is seen. Mild steatossis is seen. The features are in keeping with chronic hepatitis in a known case of hepatitis B viral infection Ishak`s necroinflammation score is 12, fibrosis stage os 4 to 5
you have to start antiviral as soon as possible, tenofovir is the best choice, your liver is severely damaged.
since also kidneys are usually damaged with severe fibrosis yo have to check creatinine while on treatment.if you see creatinine elevations entecavir is better
necroinflammation score is 12
very very high
fibrosis stage os 4 to 5
this is cirrhosis stage you have to start antiviral immediately
check also your diet, wheight and excercise because you have some fatty liver too
look for an hospital with fibroscan so that you can monitor cirrhosis/fibrosis regression during treatment and nodules if present
i also had cirrhosis last year and hve regressed it from fibrosis 16.2kpa to 9kpa by entecavir (my kidneys don t tollerate tenofovir), you are also probably on time to reverse cirrhosis, it usually take from 3 to 6 years and entecavir is the fastest to reverse it although tenofovir is a more potent antiviral
aft and ultrasound check every 6 month to prevent liver cancer since you have the highest risk for it at this stage
