Meaning of medical test

My husband has been diagnosed with chronic Hepatitis B about three years ago, following the passing of his father who had the disease which developed into liver cancer.  He has been on antiretroviral therapy eversince, with Baraclude, to be specific.  At first, his viral count continued to go down, but his last DNA test showed a slight elevation in viral count compared to the test before that.  It prompted his doctor to add another medication, Hepsera, on top of the baraclude he is already taking.  So now, he is taking two antiretroviral medications.

His doctor had him very recently take another DNA test to check the effectivity of the medications, and with it asked him to get an ultra sound, SGPT, and an AFP test.  The results of his DNA test and AFP has not yet come out, but his SGPT levels had gone up from 77 to 124.  And here is a direct quote from his ultrasound result:

"REPORTS
Liver span: 17.5cm
The liver is mildly enlarged.  Configuration and lobar proportions are within physiological range.  Parenchymal echogenicity is moderately increased with resultant increased sonic beam attenuation.  Hepatic contour is smooth and borders are fairly well delineated.  Echotexture is homogenous save for a mild to moderately echopenic, slightly heterogenous focus within the lateral segment of the left lobe.  This rather ovoid shaped focus measures 2.0x1.7x1.4 cm (LWH).  The rest of the haptic segments are unremarkable.

CONCLUSION
Moderate degree of fatty change with resultant mild increase in liver dimensions.  Solitary focus of decreased echogenicity, lateral segment, left lobe, which could represent an area of fat sparing.  It's rather ovoid shape however is atypical of fatty sparing.  Correlation with other clinical parameters or imaging modalities is suggested for definite diagnosis."

My question is, pending the result of his AFP, can someone please translate the result of the ultrasound?  I get this much:  he has fatty liver which is slightly enlarged and there is an ovoid shaped focus that could be fatty sparing, but its shape is not typical of one.  What does this indicate?  I would wish to note also that the doctor ordered the AFP test AFTER looking at the result of the ultrasound.

I hope someone can help shed light as to the likelihood of my husband's situation.  Pending the result of the AFP, i am losing sleep over this.  My husband is still 32 years old.  Pls help me, for my peace of mind.  Thank you.

The regulars there will be able to give you some feedback.  In short HepB is a very complex disease.  Your husband's presentation is not uncommon.  The Ultrasound didn't indicated anything really bad.  It just suggested additional followup for diagnostic purposes.

When you are at the HepB forum.  Go the Health Page and read the HepB Intro and Welcome page.  Then post up your husband's labs (ALTs and DNA) in chrono order (Pre and Post treatment).  Also the eAntigen and eAntibody status.
