No, antivirals like tenofovir or etencavir are drugs which block virus replication. They are not removing totally HBV but limit its damaging effects.
They have nothing to do with fatty liver. You have fatty liver when no good diet, overweight etc ..

while on antivirals we have no inflammation
of course antivirals take 1year and more to reduce inflammation, so the earlier antivirals are started the better on active hbv with lvier damage
pain has nothing to do with liver damage and even talking about it is wrong, we have fibroscan so we know everything about the liver and we have a scale from perfect liver, to liver like most population that eats crap food, to level any level of damage.
the damage starts from 7-8kpa, cirrhosis from 12.5kpa
fatty liver and inflammation amplify results, while on antivirals we have no inflammation so only fatty liver which is confirmed by ultrasound can amplify results

I started feeling pain when i started tenoforvir in 2012, i stopped medication to see if the pain will reduce yet no change.am planning make full test next month.i think it' s due to the drug fighting.but i have stopped no difference.sometimes i feel like i made a mistake starting medication.

Food and Vit D has nothing to do with pain, even from m experience, since I begin to take Vit D made it less pain. It is immune system that cause it, fighting with virus.
