Urgent please help With ur advice

Was on lam 4 d past 6 month pre treatment test hbv dna 22 million,hbeag +ve,alt  32 u/l,ast 28 u/l,after 2 months of trt hbv dna 680,000,recent test showns hbv dna 150 million,hbeag +ve,anti hbe reactive,feto protein 0.5,alt 119.6,Ast 67.5.please what should i do  to get the virus undected,any hope of getting ride of the virus,can the viral load get to undected after treatment again ,what type of anti viral should i use now,please help me,i guess my doctor make a wrong decision on the use of lamvudine,can the virus still be control.no symtops yet,ultrasound shows no damage to liver.

why did you use lam since we know from years it worsen hbv?

start tenofovir as soon as possible (viread brandname or tenvir generic)

make a combo since you are already resistant to lam with:
alinia, nitazoxanide, generic nizonide500 or nitarid 500, this is active against all viruses and lam resistant is very sensitive to alinia or interferon

entecavir, i would start combo with this too but i'd keep alinia first which is stronger, you also need combo with etv because viral load is too hugh and you must get hbvdna und as soon as possible to prevent resistance with etv

so start tenofovir+alinia+entecavir and check that hbvdna gets und by 24 weeks (6months) this way you should be safe from future resistance

find a very good liver specialist, and i'd sue old doctor if possible because lam is not a therapy for hbv anymore at least as a monotherapy (it is good with immune globulins combo after hbsag negative liver transplant only)

My new doc want me to start trt with pegylated interferon 180mcg once a week.how safe it is for me with my high viral load ? And how effective is it on hbv? Hbv dna 150 million cpcs

that's a very good choice lam resistant mutants are weaker on interferon and hbs seroconversion are higher
also take a look to nitazoxanide (alinia), 12 weeks lead in before interferon henance interferon results, alinia is like interferon very active against LAM and adv mutants and probably all other mutants as well

interferon can have heavy sides in some but try it for at least 12-24 weeks if there is no hbsag and hbvdna drop switch to ntz+tenofovir
