There is a type of Hepatitis B disease called occult hepatitis B - negative HBsAg but positive hbvdna. This fits your case, but your hbvdna is unusually high for negative HbsAg, so I would definitely test again to rule out any lab errors.
One explanation for occult HBV is mutation in the S-gene thus, the surface protein produced escapes the standard antibodies used in standard assays. There are also other explanations.

Just my understanding.

Difficult to answer.
Many patients who become HBsAg negative, with or without medication, will not develop HBsAb immediately, some may do so later. It is controversial whether to take vaccination to make HBsAb positive, because some doctors believe that these patients already have HBsAb, but just that the amount of s antibodies is not greater than the amount of s antigens, and the HBV vaccine consists of just HBsAg anyway.
Even when patients become HBsAg negative, most believe the patients will still have some residual cccDNA in the liver, but it is kept in check by the immune system so that serum hbvdna will be undetectable. This balance becomes upset when the patients need to take medication to suppress the immune system when dealing with arthritis or cancer.
In occult HBV, HBsAg is negative, but serum hbvdna is positive, but usually not as high as in your case. This is because the mutations that cause HBsAg to become undetected usually also make the virus replicate very inefficiently. In occult HBV, there is certainly no point in taking the HBV vaccine.
So I would suggest you check your HBsAg and hbvdna in another lab.
Just my opinion.

As i see, both are within range
ALT - 25.82 u/l and AST - 25.86 u/l

what is the level of hbsab?if low it cannot stop reinfection.also consider that hbsag negative in blood does not mean it is in the liver and that after making hbsag negative you need at least one year on antivirals to get stable high hbsab and neg hbvdna usually

Hi Stef...I am male. My age is ~ 40 years.

My data as follows - HBsAg positive with HBV DNA <0.04 pg/ml (latest being 109 iu/ml), HBeAg -ve ,HBeAb +ve, LFT within range, Abdomen ultrasound normal. HBsAg quantitative - 446.88 iu/ml I read a note of yours that if HBsAg is < 1000 iu/ml, then attempt can be made to make it non-reactive. If yes, please guide me. I will try my luck. Regards
