Anal issues

I am at 4 1/2 weeks into triple therapy with incivek.  I just found out that I am NDT!!!!!  I am on cloud 9!

My question to you all in forum land is how are you dealing with the anal pain, itching, generalized discomfort.  I have tried tucks, preparation H and antihistamines.  This seems to be my worst symptom so far.  I am open to any and all suggestions.

I am sitting here laughing so hard the tears  are still running down my leg.

I know you had a rough time with all those issues and I certainly don't mean to make light of it....but just struck me funny ..and I guess by what mikesimon  and nygirl posted I:m not the only one  .

Your posts are very infomative IMO ..not just the poo ones ,,,but is has got to be a bonus for members to have a pooh expert called pooh..

Hang in..and keep the sense of humor...it will help with tx..

I know, it is pretty funny that pooh and gerbils post in the anal thread.  We have to laugh at these things. It helps keep us sane.

I had pretty rough anal/rectal pain, a sickening horrible pain and itching and burning.  I'm off the Incivek now, but I was able to 90% eliminate all of the anal problems by doing two things:

1 - eat a lot of fat and oil, maybe even more than you are supposed to.  The more fat and oil you eat, the smaller the amount of Incivek that will reach your rear end (and the more will reach your liver and  stop viral replication).  I found that if I "slipped" a bit on the oil/fat, I would have much worse anal pain the next day.  Fat/oil more than doubles the absorption of Incivek.

2 - don't just wipe.  WASH!  I ended up taking a quick shower after every bowel movement, and cleaning REALLY well.  Basically I would be ready for the shower BEFORE the bowel movement, with clothes off, etc.  As soon as the bowel movement was done, and the fire-burn-itch was blazing, I would hop into the shower and get the zone CLEAN.  The pain would go away within 10 - 15 and it would stay away (until the next go-around!).  Save those bowel movements while you're at work, take them when you're at home and you can wash really well.

Good luck.  Great that you are undetectable for HCV!  The anal problems should become a much smaller issue soon after you get off the Incivek.

LOL. I know, it is sort of ironic, isnt it. But, truth be told, I picked Pooh because that was one of my cat's names.

(I know I seem to always be on the anal issue threads, but truthfully, having gone through that pain, I feel for people going through it now. And I think I figured out the solution so I post. Plus, I really do think it could be a game changer for some. I cannot imagine 12 weeks of crapping glass.)

Welcome to the forum.

I had some pretty severe anal issues, including crapping shards of glass. See my post above which lists how I dealt with the anal issues.

I really think the main thing is to keep the stools firm. I held back too because I was afraid of getting constipated, but that was a mistake. The loose stools, even the soft stools, were hell. Once I got them firmed up, the pain stopped. I did nit take a bottle of loperimide, lol. I took one at the first sign of softening stools and then if I still had more soft stools, I took another.

Anywa, I think my post above covers most of the stuff. Be sure to check with your doc before you take anything, even over the counter meds.

I started Incivek 3 days ago, adding to the peg/ribavirin after 4 shots due to an allergy I developed when I tried to treat in 2006. The nauseau/anal issues started the 2nd day, and I already feel exhausted. I have suffered with constipation issues for years, but since I started the interferon shots have had diarrhea on Saturdays after my Friday night shot. I took fiber on Tuesday, and experienced the fire breathing dragon on Wednesday! My baseline before starting any treatment was 4.6 million, my 4 week labs prior to adding Incivek had dropped to 5,577-so I am hopeful, determined, but also wonder how I'm going to get thru the Incivek part; I work fulltime, am raising my 2 grandchildren, am single and have 2 dogs and a cat-and it's already feeling overwhelming! I need to be sure and not get constipated I know, but also don't want to bring on that dragon effect! Any advice is appreciated...

everyone seems to have this issue i bought this preph anti itch cream seemed to help i used everything tucks wipes vasoline etc just keep treating the side effect it will go away after you done the incivek not immediately it just got less and less until about 6 weeks after incivek it was GONE thank god lol

Hi,

Congrats on the undetectable status! I just wanted to add that some use Balneol lotion (you can carry it with you) to assist with cleanliness. It is a soothing lotion that may be used as needed. I also used fiber to control the pooh pooh, it is safe gentle and doesn't add additional meds to the mix. Benefiber, Metamucil, eating foods that are high fiber to get your 20 to 30 grams a day.

Congratulations on being Undetectable!

It is unclear from your post if the anal pain you are having is internal pain or external pain or both. It is important because the remedies are somewhat different.

Some of us (me included) experienced severe internal rectal pain that felt like we were crapping shards of glass or crapping razor blades every time we had bowel movements. It won't matter what you use on the outside because the external meds will do nothing for this pain (which is probably caused by fissures from irritation by the meds and/or loose stools). However, what does work for the internal pain is to keep them stools quite firm, not like rocks, but definitely formed and firm. If they start to get even the least bit soft, they will really be painful. I know this seems counterintuitive if one has developed hemorrhoids, but believe me, it was the only thing that worked for me and for several others. To firm up the stools you can take loperimide tabs. Start with one or two because you do not want to get super constipated but you have to take enough to keep the stools firm. You may not need it every day. I did not take it unless I noticed the stools were getting softer. Then I took one. The other thing that helps, once you get the stools firmed up, is a Prep. H suppository. It must help some to soothe and heal the internal area.

For outside rectal pain around the anus, it is imperative that you keep the rectal/anal area spotless. Don't leave a speck of stool on the skin. For this, most of us use Huggies baby wipes with aloe and we diligently get every piece of stool off the area, no matter how many wipes it takes each time. Showers with the wate3r running over the anal area and/or tub baths melp as do sitz baths. You can use Prep. H cream if you have hemorrhoids. I also had a prescription for Xylocaine with Hydrocortiszone cream that I applied. It numbed the area and also decreased inflamation until it could heal up. Some people do use tucks as an adjunct. After awhile I switched from the Xylocaine cream to Desitin cream to coat the outside which helped protect and heal it and also stopped the itching.

Some people use other creams and balms but I do not know their names.

If you have a rash, or think the itching is due to a rash, you could add Zyrtec in the morning. It is an antihistamine but won't make you drowsy. Often the itching is due to specks of feces on the rectal area and keeping the area sparkling clean will take care of that. But if it is itching from a rash or allergy, then you may need antihistamine help (Benadryl at night and Zyrtec in the am).

Of course, be sure to discuss any over the counter meds with your physician before starting them. I also discussed them with my specialty pharmacist. Triple med treatment has so many drug interactions that one has to be very careful not to mix any that will interact. Plus, any other meds you may be on could also interact.

To repeat, I think the most important thing is to keep the stools firm. The other stuff helps, especially for the external discomfort, but keeping the stools firm is what will stop the internal pain (and it is the internal pain, crapping shards of glass, that could possibly be a game changer if not controlled rapidly).

That is great news!  I try and keep stools firm, use benefiber, and bought some non fragranced wet wipes with aloe. I do not use dry TP and that helps. To me, the biggest thing is to not get "runny"
