Couple of things......Are you treating with a knowlegable doctor ,either a gastroenterologist(experienced in the treatment of HCV) or a Hepatologist.
Quite frankly it does not sound like it,....as a geno type 1 you should have been made avail .the triple therapy of Inf/Riba  and either Victrelis or Incivek  rather just the combo of Inf. /Riba alone,
Also if this current doctor did your first PCR at week 14  this was not in your better interests,as the week 12 point is the usual cut off point for having viral load and gauging how successful you would be.

Never like to be the bearer of bad news, however given you still have a viral load at week 14 your chances of success are very low,and for your doctor to say he will continue for up to 60 weeks with this result is never recommended .

My advice would be to get referred to  a experienced specialist .right away and get their advice on whether to continue at this time with one of the new meds added  (as others have suggested ) or to stop now and give it some time and go at this  again with the proper  drug regime.

Best to you...
Will

Has your Dr. talked about triple therapy? Just wondering since if you're at 15 weeks, you were definitely in the time frame to have those available to you. How are you other labs, like your CBC? They should be monitored regularly for drops that may need to be addressed promptly.

My two cents anyway - from what you've posted, I'm not convinced your Dr. has your best interests, meaning SVR, in sight. Extending to 60 weeks and still being detectable at 15 in really old school in light of the new meds available.

If it were me, and knowing what I know now after learning so much on these forums, I would stop and regroup for now. First and foremost, I would find an new Dr. to guide my treatment plan, one that is up on all the new developments in managing HCV and liver disease.

I'm not sure what the options are as far as introducing one of the new meds at this point - hopefully someone will chime in on that question. Extending to 60 weeks of the old meds seems unwarranted to me. I did 72 a couple years back because that's what it was at the time for cirrhotics like me. But it can be a lot of wear and tear on the body unless absolutely neccesary.

What are your Dr.s plans for future PCR's if you continue?

Best wishes in moving forward, Pam

This is all very confusing but it appears that you do have detectable virus -- was that your 12-week test, and the only one?

Please do not let your doctor take you off of treatment.  Tell us your pre-treatment VL. You sound like an excellent candidate to start with the third medication - a protease inhibitor.  I bet either one could knock out those remaining virons in nothing flat.

My first treatment I had detectible virus at week 12 and relapsed but with these new PIs I was undetectible after two weeks on the PI (Victrelis).  How has your hemoglobin been?  (HGB on your CBC)

When you talk to your doctor today tell him you want to start with a protease inhibitor -- either Incevik or Victrelis.

This result was obtained by RT PCR amplification and analysis by
fluorescence hybridization with probes designed to detect a target
sequence of the HCV genome (Abbott RealTime HCV Quant ASR). This assay
has a reportable range of 20 to 10000000 IU/mL. Titers above or below
this range will not be accurately quantitated.
This assay utilizes Analyte Specific Reagents (ASRs) and has not been
cleared or approved by the U.S. Food and Drug Administration. The FDA
has determined that such clearance or approval is not necessary. ASRs
are used in many laboratory tests necessary for standard medical care
and generally do not require FDA approval. This test was developed and
its performance characteristics determined by ACL Molecular Pathology
Laboratory. This laboratory is licensed and or accredited under CLIA
and the College of American Pathologists (CAP).

Those are some weird results? It doesnt make sense. If you still have virus present, you'll probably need to add one of the new drugs (Incivek or Victreles (sp?) to the mix. Are you hep C 1a or 1b? If u have hep C, I wonder why they didn't start you with the new drugs from the start. Weird, I'd ask about that.
