Differential diagnoses

Hx:  7/99 began itching.  8/24 began flu like illness.  Aches, 101 temp., nausea, vomiting.  8/27 jaundiced. 8/29 hives began and began 4 day course of prednisone which resolved hives, aches and temp.   Now right sided tenderness, continued 100.2 fever, aches.  Prior history thyroiditis.  Had itching with it.  Two episodes meningoencephalitis, etiology unknown.  

9/6 labs:  HEPATIC PANEL:
           albumin normal
           TOTAL BILI 8.2 mg/dL
           Direct Bili 7.6 mg/dL  
           Alkaline Phosphatase 614 U/L
           AST 452 U/L
           ALT 411  U/L

           CBC
           Platelets 114
           Neutrophil 15
           Lymphocytes 77  2+ atyp
           ABS lymphocyte 7.6

           CSF
           Total Protein 57 mg/dL
