Do I need a liver biopsy

For the last one year I have had raised SGOT, SGPT  ,always between 50 and 80 units, the reference values being 46. My GGT had  been between 50 and 80 units, the reference value being 37. The CT and Ultra sound findings has been normal. ANA negative But ASMA positive, All other LFTs are normal. A diagnosis of Autoimmune has been made ,but my gastro feels that I do not need a liver biopsy at the moment and no active treatment except regular monitoring. Also could ASMA be positive in skin lesions?As I had recently been suffering from urticaria.

when in doubt push for a biopsy...it's the only way you can really tell what condition your liver is in...it would be especially necessary for making desicions reguarding waiting for treatment or going ahead with needed treatment...the worse condition your liver is in the more quickly you will want to start treatment...

but i'm just speaking in general terms. as to the reasons for your high alt and ast i would seek medical advice and if you have any doubts always get a second opinion for a more experienced hepatologist or gi.

you are right to be concerned about your high numbers and about getting a biopsy sooner rather than later and also about finding out the problem and treating it as soon as possible...

hopefully you are aware that this is a patient to patient forum with no physicians answering questions, there is one in the gastro section, though.  Your questions are better discussed with a medical professional.

The only way to tell the condition of the liver is with a biopsy.  An ultrasound will not show this.  And elevated enzymes alone will not tell the condition of your liver.  Some people have cirhossis but no elevated enzymes; others have elevated enzymes but do not have a damaged liver.  I would seek a reputable gastroenterologist who specializes in hepatitis, or a hepatologist, and request a biopsy.
