Doubt about HCV test

Hello. I'm a 49 years old bisexual man from a Mediterranean country. I recently found out that the woman with which I have sexual relationships has been HPV contagiated by me. I also take T4 137 thyroid tablets. On May 2nd 2014, I had some gay sexual games (masturbation & oral with condom and fingering without  condom, but not sex) with a 35 yars old man. 14 weeks after that event, I  had an HIV Ag/Ab Combo negative test but an Anti-HCV 1,99 test at a public hospital. Some days later, I had an Anti-HCV negative (0,5) test at a private med center. One month later (totally 18  weeks from my contact with the man, I had a third blood test: HIV negative, an Anti-HCV 2,03 and  the"double-check" (sorry, I don't know the medical term) was undefined. The same day I had an HCV RNA negative for  less than 20 IU / rnL, AST (SGOT) 18, ALT (SGPT) 20, γ-GT 14. I also have double-checked my HBc, HBsA,g HBe, HBc IgM negative. Please, could you tell me what is happening? Thank you iin advance, Mediterraneangm

Well I can't tell you what the deal was with the antibodies test other than 2 out of 3 were positive but I can tell you that you do not have HCV as indicated by the RNA test also your ast/alt are in normal range.

Is it possible for someone to become infected with HCV and then spontaneously clear the infection?

Yes. Approximately 15%–25% of persons clear the virus from their bodies without treatment and do not develop chronic infection; the reasons for this are not well known.

How soon after exposure to HCV can anti-HCV be detected?

HCV infection can be detected by anti-HCV screening tests (enzyme immunoassay) 4–10 weeks after infection. Anti-HCV can be detected in >97% of persons by 6 months after exposure.

How soon after exposure to HCV can HCV RNA be detected by PCR?

HCV RNA appears in blood and can be detected as early as 2–3 weeks after infection.

A reactive antibody test does not necessarily mean a person still has Hepatitis C. Once people have been infected, they will always have antibodies in their blood. This is true if even if they have cleared the Hepatitis C virus. A reactive antibody test requires an additional, follow-up test to determine if a person is currently infected with Hepatitis C.

NUCLEIC ACID DETECTION

The diagnosis of HCV infection can also be made through detection of HCV RNA using reverse transcriptase polymerase chain reaction (RT-PCR) techniques (Figure 1). HCV RNA can be detected within one to two weeks after exposure to the virus, weeks before the onset of ALT elevations or the appearance of anti-HCV. In some patients, the detection of HCV RNA may be the only evidence of HCV infection.

If your still not cool with the testing that has been done, I would suggest that you wait a month and repeat the RNA test, at this point the antibodies testing is pointless.

No worries though, IMHO the chance you come back positive are very slim to none
