Genotype 1a, 1b, comparison??

My question is the difference between the two subs of genotype one? Did genotype 1b start as an A, and mutate to a B? Does anybody know the difference between the two? And also,I see from people on this forum, and many on other forums, that it seems like the 1b's SVR% are slightly higher than 1a's.. I have seen alot of 1a's relaspe or not respond.. Has anyone else seen the differences in response betwen the two, or are they the same? Has there ever been a study on SVR% geared at the sub categories of genotypes, or do they always classify them as one??Most of the geno 1's that posted here, of SVR, where 1b's..

Man, read some of the posts from the weekend , and you guys have been on the brutal on the attacks. Figured I'd post something, as I'm everyone's favorite target, and let some of the others take a breather..Besides, I don't think Rev has been himself, since I left..

that is one question i have been pondering about often, and last time I asked(months ago) there was no answer. I have found nothing myself, since many studies don't even breakdown by genotypes, I doubt if any wants to breakdown subgenos.

All I can provide at this time is that in pre-pegylated INF times, there was enough of a difference in SVR rates between a 1a and 1b to make note of it. With the advent of pegINF any meaningful differences in response to treatment has mostly evaporated.

There is indeed a difference between 1a and 1b.  The main difference is genetic.  Dr. Blatt explained it this way, "There is a 98% similarity between humans and chimpanzees, but what a difference that 2% makes."  Dr. Blatt went on to say that the genomic differences between genotypes and subtypes are broad enough to consider all of the hepC strains as different viruses.

Genotype 1b is considered somewhat more difficult than 1a.  1b is known for its penchant for damaging the mitochondria of hepatocytes.  This can set off a more damaging immune response.

The mitochondria store the oxygen that powers the cell.  When 1b damages the mitochondria, diogin is expressed into the cytoplasm.  This causes the disorganization of cytochrome 3 inside the mitochondria.  Once the disorganization occurs, the mitochondrial wall becomes permeable, releasing oxygen into the cytoplasm.  The oxygen binds with hydrogen and becomes hydrogen peroxide, which causes extreme oxidative stress.  Cytochrome 3 then sets off caspases 3-6-9 which call for the apoptosis induction factor (AIF) to travel from the cell wall into the nucleus of the cell.  This can result in apoptosis (programmed cell death), and necrotic activity, all of which come from the immune system.  Since 1b causes the most damage to the mitochondria, it may also cause the most destructive immune response via active genetic signaling.  In other words, viral genetics change host genetics, which may add to viral entrenchment.

The other oddity about 1b is that transfusion recipients tend to have the 1b strain.  When you think about how many 1a's may have donated blood, we may be looking at a mutation that occurs sometime between blood donation and transfusion.

Also, some studies indicate that 1b's may be the true PKR binders, although there is some debate as to whether or not this phenomenon actually exists.  The theory is that NS5A causes a mutation in E2, which makes E2 genetically similar to PKR - SO similar, that E2 binds with PKR and prevents its phosphorylation, which is necessary to the antiviral response.

One solution might be the addition of beta IFN, which increases the expression of PKR, especially in LVL patients.

Thanks, do you have alink to where that info came from. Good stuff, exactly what I was looking for.. So 1b is actually exactly as I thought, the 1a geno that mutated somehow and got stonger. That is promising, as I have seen alot of 1b's clear tx, I think Indy was a 1b also..

with the high level of contempt you blatently display for those that struggle with poverty, abuse, drug addiction, alcoholism and mental illness, i am not surprised you wants are not being met at this time.  i then suggested the following:

perhaps a volunteer posisiton at a street level facility that offers support, help, guidance and just listens to those that struggle with the very issues you seem to fear so deeply and thus hold those souls in great contempt and dispise so....you may be fortunate to learn from them and their experiences, you might gain some valuable insight into the human spirit, learn wonderful lessons from those you dispise, and maybe look at why you REALLY feel that level of disgust for souls who have been dealt a different reality from your cozy privelidged one.  it is something you may learn from and benifit from greatly.  then maybe you will be more open to receive the gifts you seek.

I just wanted to say hello to everyone. I somehow stumbled across MHI and am really pleased with what I have been reading. I am a 52 uear old male from Kansas that has HCV and cirrhossis.I have tried the combo treatment a couple of years ago and got nothing but sick. I have an appt. with another specialist coming up in the middle of Aug. My regular MD suggested I see one again. I was diagnosed with HCV in 1998 and had a biopsy done the same year to confirm it and the damage done to my liver. I had been a very heavy drinker and drug user for years. I have another interesting problem to go along with this. I also have a seizure disorder since 1990 and have been taking Dilantin ever since. I recieved a letter from my doctor in 1998 stating that I should stop my medication immediately (Dilantin) because it is bad on the liver. We found out that I could not stay off of the Dilantin for long. This is how I found out about the HCV. I have seen some very good specalists in the past and so far they have told me the same thing; that I am between a rock and a hard place. I haven't seen a specialist in awhile and my yellow tint seems to be a little darker. I didn't mean to take up a lot of time,I just want to say thank you.

I'm away from forum for 9 hours having a bx done and look what happens ..major hissy fits additional Pi**ing and moaning matches and my pal snook is back. what's next Indy being drawn and quatered for supposed sexist remarks? All the business ventures in cohorts with Chevy have been in jest I caught that early It has been truly humourous I suppose that I must of said some socalled sexist comments in my banter with Chevy also, but haven't been called on it.Chev and I flirt with each other as often as possible, for no other reason that we like to and enjoy each others take on things Just like her and Indy My gosh, with all the senitive egos  around, lighten up It's only fun.
I quess next comes the cat fight complete with the bit** slapping. Indy and I are Hosting the event and Chevy will be selling tickets Buy early because we expect a large crowd and many have free passes already, so seating will be 1st come 1st served. We will consider jello wrestling, and wet tshirt contest and take additional suggestions for future events.

Potassium bromide and sodium bromide are both very effective anti-seizure treatments that cause NO liver damage.  They were popular during the 1800's, but are rarely used today because they've been replaced by more modern medicines.  You may want to ask your doctor about this.

You see I'm perfectly content to put myself, all my loved ones, all my worries and woes in God's hands, and accept that whatever happens was His plan. Nothing, absolutely nothing, happens in God's world by mistake. That includes you getting HCV. I'm not saying that ANYONE deserves this disease, but for whatever reason, God has decided to give us this challenge. I look at it as another chance to prove that I am worthy of His love. How do you see it? As a punishment? Then that is what it will be. What doesn't kill me makes me stronger. It makes you bitter and vindictive. Which one of us is going to have an easier recovery and a content life despite of HCV?    Joni

Just because I've never had a drug problem doesn't mean I don't have compassion for people who do.  But I also don't think drug addicts, past or current, are "COOL".  I think that's the comment that has branded me as being insensitive.  Well, so be it.  

I don't view my diagnosis as a punishment, by any means.  In fact, instead of saying "Why me?"....I say: "Why not me?"  I've even posted this personal philosophy to others who have slipped into self-pitying thinking.

Some playful banter between Scott and I last week has obviously been taken way too seriously.  I'm not a "rich, priviledged" person! In fact, I'm the only member of my family to graduate from college and I paid for every penny of it myself.  I've worked hard and have achieved some modest success in my career.  But I'd trade it all for a child and the experience of being a parent.  Unfortunately, I may never get that chance.  But even then, I trust God (oops...sorry...didn't mean to bring ups the "G" word") and his plan for my life.

You see, I'm religious and spiritual....and a whole lot more tolerant than many of the folks on this board who are bashing me.

no, actually amoung the many comments you have made that have somehow left most feeling you are 'insensitive' (as you say), i think the one you said about a doctor suggesting you had ever done drugs, and your reply about the fact that "nothing about you LOOKED like you had ever done drugs" , well we all wanted to know exactly what someone that HAD done drugs "looked like" in your opinion...and then you went on to debase those who had ever done drugs at all, as "dirty, filthy, drug addict junkies" and then attempted to back peddled at the back lash of that comment, you still used reference to those as 'dirty junkies', thus negating entirely, your attempt at an apology...THAT was what rattled most of us monkeys, this one inparticular.
