Genotype 1b is considered somewhat more difficult than 1a. 1b is known for its penchant for damaging the mitochondria of hepatocytes. This can set off a more damaging immune response.
The mitochondria store the oxygen that powers the cell. When 1b damages the mitochondria, diogin is expressed into the cytoplasm. This causes the disorganization of cytochrome 3 inside the mitochondria. Once the disorganization occurs, the mitochondrial wall becomes permeable, releasing oxygen into the cytoplasm. The oxygen binds with hydrogen and becomes hydrogen peroxide, which causes extreme oxidative stress. Cytochrome 3 then sets off caspases 3-6-9 which call for the apoptosis induction factor (AIF) to travel from the cell wall into the nucleus of the cell. This can result in apoptosis (programmed cell death), and necrotic activity, all of which come from the immune system. Since 1b causes the most damage to the mitochondria, it may also cause the most destructive immune response via active genetic signaling. In other words, viral genetics change host genetics, which may add to viral entrenchment.
The other oddity about 1b is that transfusion recipients tend to have the 1b strain. When you think about how many 1a's may have donated blood, we may be looking at a mutation that occurs sometime between blood donation and transfusion.
Also, some studies indicate that 1b's may be the true PKR binders, although there is some debate as to whether or not this phenomenon actually exists. The theory is that NS5A causes a mutation in E2, which makes E2 genetically similar to PKR - SO similar, that E2 binds with PKR and prevents its phosphorylation, which is necessary to the antiviral response.
One solution might be the addition of beta IFN, which increases the expression of PKR, especially in LVL patients.
After reading some of the weekend posts, it's difficult to distinguish which species some of us belong to!

with the high level of contempt you blatently display for those that struggle with poverty, abuse, drug addiction, alcoholism and mental illness, i am not surprised you wants are not being met at this time. i then suggested the following:
perhaps a volunteer posisiton at a street level facility that offers support, help, guidance and just listens to those that struggle with the very issues you seem to fear so deeply and thus hold those souls in great contempt and dispise so....you may be fortunate to learn from them and their experiences, you might gain some valuable insight into the human spirit, learn wonderful lessons from those you dispise, and maybe look at why you REALLY feel that level of disgust for souls who have been dealt a different reality from your cozy privelidged one. it is something you may learn from and benifit from greatly. then maybe you will be more open to receive the gifts you seek.

I'm away from forum for 9 hours having a bx done and look what happens ..major hissy fits additional Pi**ing and moaning matches and my pal snook is back. what's next Indy being drawn and quatered for supposed sexist remarks? All the business ventures in cohorts with Chevy have been in jest I caught that early It has been truly humourous I suppose that I must of said some socalled sexist comments in my banter with Chevy also, but haven't been called on it.Chev and I flirt with each other as often as possible, for no other reason that we like to and enjoy each others take on things Just like her and Indy My gosh, with all the senitive egos around, lighten up It's only fun.
I quess next comes the cat fight complete with the bit** slapping. Indy and I are Hosting the event and Chevy will be selling tickets Buy early because we expect a large crowd and many have free passes already, so seating will be 1st come 1st served. We will consider jello wrestling, and wet tshirt contest and take additional suggestions for future events.

You see I'm perfectly content to put myself, all my loved ones, all my worries and woes in God's hands, and accept that whatever happens was His plan. Nothing, absolutely nothing, happens in God's world by mistake. That includes you getting HCV. I'm not saying that ANYONE deserves this disease, but for whatever reason, God has decided to give us this challenge. I look at it as another chance to prove that I am worthy of His love. How do you see it? As a punishment? Then that is what it will be. What doesn't kill me makes me stronger. It makes you bitter and vindictive. Which one of us is going to have an easier recovery and a content life despite of HCV? Joni

I don't view my diagnosis as a punishment, by any means. In fact, instead of saying "Why me?"....I say: "Why not me?" I've even posted this personal philosophy to others who have slipped into self-pitying thinking.
Some playful banter between Scott and I last week has obviously been taken way too seriously. I'm not a "rich, priviledged" person! In fact, I'm the only member of my family to graduate from college and I paid for every penny of it myself. I've worked hard and have achieved some modest success in my career. But I'd trade it all for a child and the experience of being a parent. Unfortunately, I may never get that chance. But even then, I trust God (oops...sorry...didn't mean to bring ups the "G" word") and his plan for my life.
You see, I'm religious and spiritual....and a whole lot more tolerant than many of the folks on this board who are bashing me.
