This member of our forum was a genotype 3, had compensated cirrhosis, treated several times and finally acheived SVR. He tweaked his treatment and the results were successful. Perhaps this information will be helpful. His screen name is gauf.

Geno 3a. 2x relapser, stage 4 compensated. Biopsy in '05 Stage 2/3 Downgraded to 3/4 with blood markers, etc. VL 850,000. 12 week treat with pegasis and 800 Riba. Developed serious infection and anemia at 11 wks and taken off 1 week early. UND at 12 wks. 6 mos post VL 2 ... [More] 50,000.
Retreated Fall '06 Pegintron and 1000 Riba. 24 weeks 100% compliant. Anemia treated with procrit. UND at 12 wks and EOT. 6 mos post VL 4 million. Doc ready to treat again fall '07. Planned pegasis and 1000 Riba for 48 wks. Got second opinion from Hepotologist. Downgraded to stage 4/A portal hypertension but compensated. Diagnosed with steatosis as well. (probable nafld) Will up Riba to weight based. Need to get weight down before starting treatment #3. Platelets and WBC still low 12 mos post. Update: Next tox. will predose Riba for 2 weeks, and treat wiith pegasys 180 every 5 days for first 4 weeks then weekly. Riba 1200 throughout. Alinia 500 mg 2x daily for at least first 3 mos. PPC 900 mg x2 daily throughout.
As far as the supplements go on tox:
I WILL take 1 dose NAC/Vitc, PPC, TMG, ALA, Vit D, lactulose and lactobacillus GG in the middle of the day to avoid any obsorption problems with the Riba.
I will NOT take Milk thistle or curcumin or resveratrol or green tea. No anti inflammatories at all.
I will add lactoferrin near EOT. It significantly boosts the immune system, I think it a prudent addition to SOC at the END of treatment. Instead of stopping cold turkey at EOT on the soc drugs (like I did on the last 2 txs'), I plan to taper down and give my own immune system some help coming back online. As soon as I start to taper, I will add the lactoferrin to help. Camel Lactoferrin is 30x stronger than bovine, so I will try that. HR thinks this plan cerrtainly couldn't hurt, and may just help.
(update 3/08) Endoscopy revealed 2 varices (moderately severe non bleeding) in the lower GI tract.
(update 4/15/08) latest bloodwork WBC normal. ALT normal. AST 65. Platelets still low. VL= 3.14 mil.
(update 5/08) Begian tox 3# Made some changes in plan. See journal entry 3rd tox begins.
