What happens next?

I've come up HCV reactive with an HCV ratio of 31.3
And these are the only 2 things I see on the electronic display from my lab via
my md office.

My md is on vacation.  What is the next test?  Or the next step?

It tells the provider that you have been exposed to Hepatitis C sometime in your life. 20% of people clear the virus on their own but will carry the antibodies for rest of life. PCR test will confirm if you have an active virus or hopefully you were one of the lucky few that cleared the virus on their own.

Also with a ratio of 31.3 the odds of antibody test being a false positive is only 5% of less.

The next step is to look for the actual virus in the bloodstream, using a test called PCR.
