Hello, identified hepatitis C genotype 3a/b.Nigde not find information on this genotype. What is the duration of the treatment given genotype? Thanks

Hi!  I went through Tx a year ago and I had Geno 3a.  I did 24 weeks of Pegasys/Ribavirin and was a rapid responder (RVR) at four weeks.  I ended up clearing the virus and both my 3 month and 6 month tests had me cured :)  Yay!  It was a long road, but well worth it.... :)

There are people who just genotype 3a / b? Very conflicting opinions. Some say that it is necessary to treat 1 year, and the other 6 months?

I was a geno 3 dont no if a or b or both.

I am now SVR but it took two treatments 24 weeks + 48weeks.
If I have had enough meds and, or time first tx it would probably had been enough with one.

My adwise to you is get a skilled doc preferably a hepatologist who is up to date and know his business.

In order to know how long you need to treat depends on how fast you get to UND.
If you have a low baseline Viral load and a rapid response UND before 4 weeks
you might only have to treat for 16 weeks.
Therefore its importent to  early get as many PCR tests as possible one after two weeks is good but absolutely one after 4 weeks of treatment.

I only got one after 12 weeks on my first tx.

To be honest, I have never heard so anyone with a cross infection of 3a & 3b.  Of course that is not to say there are not other people in the same situation as you, just that I have not heard of them as yet.  I would suggest that you consider yourself a Geno 3 and imagine that you would follow the treatment plan for such.  These days people are having treatment length tailored according to their intitial responses to treatment.  Sometimes Geno 3 is considered easy to get rid of but in some people it has been proving more difficult and those folks have been treated as Geno 1s.

There are also new Ploymerase Inhinbitors in developement that appear to be having good results with Geno 3.

Genotype is usually treated for 24 weeks.

There is a catch though.

Once you start treating you have your Viral Load checked. If you still have virus in your blood after 4 weeks of treating, you should extend treatment. How long to treat would then be determined from when you do NOT have any virus left.

An example.... If you do not have any viral load left after 8 weeks of treatment, you treat for 32 weeks.... that is 8 + 24 = 32

If you still have viral load after 12 weeks of treatment, you stop treatment. Then it didn't work and one will have to find out why and try to treat again, with different doses of the medication.

I'm with comeagain as far as thinking a 4 week viral load test would be the determining factor on whether to treat as a geno 3. Anything other than undetectable at that point would require the opinions of two or three experienced specialists and even then you'd still be playing the odds depending on if and when you hit UND. I thought I had a weird case with geno 3e but your the only 3 a/b I've ever heard of.
