hcv tx low hemoglobins

When should I go to the hospital? I amconcerned because hemoglobins were 10.7 a week agoon Monday. 3 days ago 8.6. How fast will it continue to drop? I did procrit shot yesterday. No change in way I feel. Already had transfusion sept 5, 2 units cuz it dropped from 9 to 8.4 in a day.

I wish blood counts, etc. were standardized. I'm in Canada and we don't measure HG the same way.

My count is really low (99), and my nurse recommended last week that I drop a riba - so I'd be down to 600 mg a day from 800.

I'm going to cut back on work instead. Just three weeks to go (19 sleeps!) - I should be able to pull this off on a full dose.

But wow - this is definitely the worst I've felt so far.

theres lots of symptoms on tx....my sore throat didn't go away until post tx....funny thing was it was on one side of my throat up into my ear...also constant ringing...i would be careful not to lower doses when having hgb in the 9s...i have seen folks try to feel too good only to fail tx....good luck....billy

I would not to dose reduce either. None of us really does. Kudos to your doctor for keeping you on your normal dose of Riba. I just hope you can find the right dose and frequency of Procrit rather than intermittent use and having to go to the ER on the weekend for a transfusion.

Keep in mind that weekly Procrit means weekly CBCs so your doctor is wise to advise you to come in on Monday. I how you two can establish a treatment regimen that works for you.

The symptoms of a cold or even flu-like symptoms are characteristic of the meds. You might want to try Tylenol for your headaches :)

I ended up goin to er saturday. They took me back pretty quick. Hemoglobins had jumped back up to 9. However my proteins and magnesium was really low. Everything is in the too low or too high range. My dr was on call at hospital so she called in and said to do fluids and to do magnesium. Sent me hm said to
come to office Monday instead of next week. We lowered riba once while on incivek but it isn't a recommended treatment to lower it.
I am on procrirt intermittently. Probly be permanent now.
I hv had mild nose bleeds every day too. I keep getting huge headaches, even migraine meds aren't unfortunately
However, now unfortunately, I think I am gettin sick. I hv sore throat, feel congested and my head hurts like it does with a cold. I am probly gettin infection of some kind. Ugh.

Dose compliance is directly correlated to SVR succes, and that is the reason the whole notion of "rescue" drugs was invented.  The goal is the stay on effective dosing of the hepc drugs.  Dose reduction is the very last thing to do, not the first trick pulled out of the bag, IMO.

My HGB tanked on triple therapy. It was in the 8's, it would go up with a transfusion and then drop right back down.  I was dizzy, I would faint if I stoop up too quick and started walking, I had nose bleeds that wouldn't quit.  I had regular transfusions every two weeks for a while, one after going to the ER.

I ultimately failed treatment (viral load started increasing, indicating a quickly growing strain of resistance mutations), but I know it was not due to anything I could have done -- I stayed 100% compliant.

I've been off all meds 10 months, and my HGB just now is north of 13.  Low normal for my test is 13.5.

I know it's tough, but hang in there!

My doctor  did not want to run the risk of a reduction in riba dose.  I had Procrit for the rest of treatment(after week 10) It helped.  I think there were only 2 weeks when I did not take the Procrit (I had blood work ever week to monitor).   I didn't have to take a break walking up one flight of stairs anymore ...lol.  I did learn to take it easy.... The nurse told me (before Procrit) that if I felt like I was about to faint and/or if my lips turned blue, to go to the ER.  Hope this helps.  Keep up the great fight!

I just read the other post where you asked this question and Hector makes a good point. At 1200 mg Riba a day I am surprised you have had transfusions and Prcocrit without first reducing your Ribavirin dosage.

With 34 weeks to go you might want to speak up to your doctor about how to manage your Hgb in the long run. I am not a huge supporter of Riba Dose reduction but the studies demonstrate it does not impede SVR. Even in my case the first line of defense was to lower my Riba for a few days until I got to feeling better.The Procrit was on order so I knew it would not be indefinite. I was able to go to full Riba dose once my Hgb rebounded a little. It still flip-flops a little but not the dramatic his and lows you are having.

Call your doctor's office tomorrow! You might want to consider calling your doctor tomorrow (as in lickety split). You do not want to worry and wonder about this all weekend.

OOps, piglet, just saw yr first post, from earlier today.
  The Procrit tends to take as long as 3 weeks to take affect, although it's not affective for everybody.
  We can also get used to low hgb, but not in your case, becuz your hgb is dropping quickly, as opposed to gradually.
  I know that below 8., is when a transfusion is given. At least, if you went to the ER, they would test your hgb again~  good luck, and hang in there. Lots of us have had to go to the ER, so dont worry about that, if your instinct tells you to go, just have your husband take you. You are having an emergency, that's what the ER is for

I dropped first time on my lasy week of incivek. Was told I should be okay after transfusion. However its been 3 weeks of almost normal and boom down again to 8.6 on Monday 9-24. This is thursday. Did 40,000 procrit yesterday. Same dose of riba hvnt changed that. 3 pills am, 3 pills evening. I hv headache that nothing is helping, muscles hurt, short of breath, having dizziness and lightheaded, hv a weird cough.. I'm worried but trying not to make my hubby take me to er. I hate stressing him out. I was hoping worst was over when I finished 3 a day "fat" pills (incivek), but apparantly its the riba causing hgb to b low. Peg shots make me feel like I hv flu for 3-4 days at a time. I can tolerate that but blood count goin down is hard to live with. I hv 34 weeks to go.

Hi what did your doctor say when you got the results?  I went from 14.8 to 9.7 in a short time, felt awful, could not catch my breath.  My doctor said looking at all numbers I was fine for being on tx, he told me to lay down.  Not sure why he would not give me rescue drugs..  Finally my riba was lowered at 19 weeks and I started to rebound a bit
I hope you feel better soon.  Procrit takes a while to work.  If you start to feel worse I would call the doc, go to the hospital.

my hgb was in the 8s most of tx...i was told no transfusions until in the 7s....watch out you don't try to raise your hgb up too much by lowering ribas..i have seen folks get their hgb up to high 9s only to relapse....low hgb can be good

3 days ago you were 8.6 and you administered a shot of Procrit today?
I saw you finished Incivek 12 September. My Hgb dropped after the Incivek period as well. It managed to drop so low since I went from having weekly labs to once every few weeks. I have weekly labs now - hopefully you will have a CBC once a week as well.

How many units? 40,000?
What is your dose of Ribavarin? Did you lower it?
Is a week ago on Monday the 17th (???)

It takes a couple of weeks before Procrit kicks in an makes a noticeable difference in your labs and how you feel. If you Riba dose reduced you may feel better a little sooner but even this varies from person to person. I ended up needed a Procrit dose increase from 40K to 60K but it took a little experimenting to determine the right dose of Riba and the frequency of Procrit.

Hopefully others will chime in as I am unsure about dropping from 9 to 8.4 in one day.
