high ammonia levels

Yes it does. High levels of ammonia cause hepatic encephalopathy which in turn can cause confusion, sleepiness and forgetfulness. There are medications to help lower those levels. Speak to your hep doc.

Not necessarily and most likely not in your situation. 145 is not that high and ammonia tests are famously innacurate for the purpose of diagnosis. It can be more useful after several tests have been run.  An ammonia test also does not have the ability to detect other toxins which may be contributing to, or could even be the sole cause of any encephalopathy in a patient.
