A team of US researchers say that an obsolete drug, known as 'clemizole hydrochloride’ may be effective against the liver disease 'Hepatitis C'. The team has made two new discoveries which could help in fighting against the Hepatitis C virus.

Researchers from the Stanford University discovered a protein called NS4B, which plays an important role in binding some of the genetic material, or RNA and allows the hepatitis C virus to replicate.

They also discovered old drug ‘clemizole hydrochloride’ (previously used as anti-itching medication) could slow down that protein, which in turn will decrease the virus replication tenfold with no apparent harm to infected liver like cells.

The team says that as the drug has been already used by many people and hence it is safe for human testing. Glenn added that old drug ‘clemizole hydrochloride’ could be become an essential component in a new class of multi-drug treatments for liver disease.

We recently identified a compound, clemizole hydrochloride, that inhibits NS4B’s RNA binding and hepatitis C virus (HCV) replication.

Although significant, clemizole’s antiviral effect is moderate
(50% effective concentration of 8 muM against an HCV genotype 2a clone)

We hypothesized that the combination of clemizole with other anti-HCV agents can increase the antiviral effect over that achieved with each drug alone and could also decrease the emergence of viral resistance.

Methods
.
Luciferase reporter-linked HCV replication assays were used to study the antiviral effects of drug combinations that included clemizole. Data were analyzed using Loewe additivity and Bliss independence models for synergy, and resistance studies were performed using HCV colony formation assays.

Results
.
Clemizole’s antiviral effect was highly synergistic with the HCV protease inhibitors SCH503034 and VX950, without toxicity.

In contrast, combinations of clemizole with either interferon, ribavirin, or the nucleoside (NM283) and nonnucleoside (HCV796) HCV polymerase inhibitors were additive.

Furthermore, combination of clemizole with SCH503034/Boceprevir decreased the frequency of drug-resistant mutants, compared with treatment with either drug alone. Finally, no cross-resistance to clemizole of SCH503034/Boceprevir -resistant mutants (or vice versa) was observed.

Conclusions

Clemizole can yield high-level synergy with the protease inhibitor class. Inclusion of clemizole in future anti-HCV cocktails can represent an attractive paradigm for increasing current virologic response rates.

This looks tremendously exciting.  Here is an approved drug, and probably a low cost drug that can be used to possibly add one more log drop in initial response.

The faster one gets to undetectable, the less likely one will develop resistance, and possibly, the shorter the treatment time required.

It is November now.....I wonder if this will be addressed at the AASLD?  We may soon get more details.

That sure sounds promising.  I hope to see more about this in the future.  I wonder if it has just been laying dormant for 2 years.
