How do posts that are 6 years old appear on this feed?

Is it just that someone runs across an old post, replies to it and it pushes it to the top again?

Some of the other forums on MH actually have an icon at the end of the old message that will not allow you to reply.  It asks if you want to continue the discussion, you click it and it takes you to a new question page.
I  I have seen this on the Breast Cancer page, I think it is a good idea.

I see. I usually only read the most recent posts so am always a little thrown when I see one at the top and realize it's super old
Since Hep C treatment is advancing at break neck speed, it would be sad if someone were to read an old post and not follow up with more recent data
