Protease Inhibitors

Keep in mind that in order for protease inhibitors to be effective, they need to be taken on time, and at the right dose. It is important to tell your doctor if you are having trouble sticking to schedule because of side effects. There are ways to manage these side effects. Your doctor may reduce the dose, or switch you to another drug.

MARCA...i have to say that with the extra BOC pills ...its a full time job now...im serious....i have to take the pilss as close as i can to the sceduled times...right to the minute....so i have to be aware all the time...r

1.Riba at 6am------3pills
2.Boc at 7 am------4pills
3.Boc at 3 pm------4 pills
4.Riba at 6 pm-----3 pills
5.Boc at 11 pm----4 pills

Next-Generation HCV Protease Inhibitor SCH 900518

As part of its long-term commitment to hepatitis C therapy, Schering-Plough also is developing SCH 900518 ("518"), a next-generation HCV protease inhibitor. A Phase IIa study with 518, known as the NEXT-1 study, is currently ongoing. The company said that 518 has been shown to be 10 times more potent in-vitro than other protease inhibitors currently in Phase III development and has the potential for once daily dosing. 518 also has shown decreased emergence of resistance in vitro. Given its pharmacokinetic (PK) profile, the company anticipates that 518 may be active against some HCV strains that are resistant to other protease inhibitors. Phase I proof of concept studies with 518 in treatment-naive patients and those who failed prior treatment, both as monotherapy and in combination with peginterferon (without ribavirin), demonstrated enhanced antiviral activity, with up to 4 log10 and 5 log10 decreases in circulating HCV, respectively

patients who received the boceprevir P/R lead-in regimen and had rapid virologic response (RVR), defined as undetectable virus (HCV-RNA) in plasma after 4 weeks of boceprevir treatment, SVR was 94 percent in the 48 week regimen and 82 percent in the 28-week regimen. RVR has been shown to be a reliable predictor for achieving SVR. These final results are from the HCV SPRINT-1 study in 595 treatment-naive patients with chronic hepatitis C virus (HCV) genotype 1.

dude, you've been a busy boy....I'm lovin it.
but I gotta say 2 things...

1. I wish you'd put stuff like this on the medical side of the forum, new research rightfully belongs where treaters will notice it.
Like I'm on Alinia now, because it was posted and discussed on the med side...but I seldom get over to this the "SOCIAL side" of the forum...and so I would have missed the opportunity to learn if you follow, and would'nt be on Alinia right now if it had been buried on the social side.  (which it seems these days folks have forgotten why the two sides were created even. 1 is to ask/discuss anything medically relavant or a medical problem,the social is just to shoot the breeze and lighten up with the non-medical side of all our lives.

2..OK I'm just throwing tomatoes in your iron pan now, but I don't like where this protein study is going. OK, sure would be nice to know if it was hopeless...BUT  they have a 90% accuracy.....and that ain't good enough.
  
That means that 10 out of every 100 people could be turned away,
denied or told they can't treat...
who actually could have treated successfully.  
So while I applaude the effort to get there, the predictives would need to be above 99% accuracy to satisfy me. People are not statistics.  

ps. they tried that on me...rattled off the statistics to get me to quit...but now I've been UND for almost a year....so sc.rew their 3% odds....I've beat em, and a lot of others did too...and anyway, I've given myself a year of not writhing in pain and itching like a bee's nest, so even if I don't SVR my liver got a rest on tx.....time to get affairs in order etc....there are benefits......
I'm way paranoid about any time the insurances get little tidbits to help them deny coverage, I've got one friend and one family member given less then a 3% chance 15 yrs ago...that are still walking around today....had they been denied based on predictive, as the study suggest could and possibly should happen....would they be here, no, they would not have done the chemo, and would be long gone....and also, they say quote "so that they can choose other options"...WHAT other options??  Besides SOC you've got trials (which are called TRIALS for a reason...i.e. they are far from ideal....and transplants...and that list is growing to where only 1 in 18 will get a liver in another 5 years who goes on the list.....some "better options"??  yeah this I'd love to see......
so you see my points here hopefully..it's easy for a non-infected medical to say something trite like...choose better options, but until there actually are such things it does the sufferer a great injustice to even suggest such things. My view is refusal to treat based on statistics is a form of discrimination. They don't know who will really do well to nearly the extent they think...because it's not all about protein chains anyway, it's about lipid chains and alice in chains, and whether you take all your meds and a hundred other variable besides that have yet to be explored....sheesh...mama I need to stop now.

Who Will Respond to Hepatitis C Treatment

With 90 percent accuracy, three clusters of proteins in the blood samples were found to predict who would respond to therapy and who would not. Dubbed a ‘protein signature,’ those with the cluster of proteins indicative of non-responders could be directed towards other treatment approaches. Thus, individuals with this non-responder protein signature could be saved from many months of side-effect ridden and costly treatment with interferon and ribavirin

Hep C Study Reveals Taribavirin a Good Alternative to Ribavirin


At the end of a 48-week, Phase IIb study, taribavirin shows similar effectiveness as ribavirin in reducing Hepatitis C viral load. However, participants taking taribavirin had a significantly lower rate of anemia.
