Dear Dr Dietrich,

I have had a hepatitis in 1976 - then they only differenciated A and B and I was diagnosed non A nonB - some 30 years later after thinking I had rheumatism I found out that I has Hep C still active and acute (genotype 1). I started treatment in the summer of 2006 (peginterferon 180 and ribavrin 1000) and spontaneously responded - my viral load was eradicated after 3months and remained negative, but we had to cut down the interferon to 135 because of my low platelets. I was treated until october 2007 and a couple of weeks later I relapsed. My status today (from france):
I recently did an Actitest Fibrotest : score F3 (0;68) A1 (0.30)
Alpha Macroglobilin (g/l) 3,50
Apolipoprotein A1 (g/l) 1,53
Bilirubin (microMol/l)
Haptaglobin (g/l) 0,20
Gamma GT (Ul/l) 30
ALAT (Ul/l) 34
I tested positive on rheumatoid factor 29,7 UI/ml
Waler/rose positive 10UI/ml
Antinuclear antibodies positive (80)
and Cryoglobuline after 7days at 4°C is positive too (weak)
my viral load is at 681 673 UIVHC/ml (5.8 log)

I will go to the hospital on june 30th for a biopsy, endoscopy and colonoscopy -

I am quite ready to try treatment again but my doctor is seemingly reticent - I don't know what to do

Too early to tell what to do until we have biopsy back. However if your platelets were low you may have significant liver disease. You will also be a good candidate for the new drugs for HCV, telapravir being the first one on the market or the mostly likely to have a clinical trial near you. Good luck!
