Cold Sores and Oral Sex, Type of Physician

I am fairly certain that I have contacted HSV-1 from my wife.  We have been married for well over 30 years, and engaged in oral sex while she had a cold sore.  We did not know that this could be a result, and, frankly, I thought after 30 years we had alrady shared everything we possibly could.  We have discussed the situation, and concluded this is the only explanation.
I need to see a physician for treatment.  Is there a specialist I should seek out, Dermatologist, Urologist?
We have refrained from sex since the outbreak.  What concerns should we have regarding future sexual relations? What would be the course of treatment?

How do I proceed?

if it's been more than a week, frankly treatment isn't going to do much.  I would follow up on this monday though and both of you get a type specific herpes igg blood test just to make sure that hsv1 really is the culprit.  Your regular doctor can do all this for you - no need for any specialist.

If you have hsv1 genitally and she has it orally, the only precaution is what you are already doing - avoid sex whenever you have anything going on genitally.  She has significant protection from contracting hsv1 genitally from you.  Hsv1 genitally typically doesn't reoccur very often and also doesn't shed too often.
