I think the chances that you will test positive on the western blot are very high indeed, more than 95%. However, I have, in my career, had two people with values higher than 4 that have no confirmed on the western blot so maybe worth doing? Remember, you can have outbreaks anywhere from your waist to the middle of your thigh.
The routine test you did to detect herpes infection is not terrific at detecting HSV 1.
It is unlikely that someone would acquire HSV 2 from you by giving you oral sex but it is possible, for sure. If you are concerned about transmission, I would advise you be on daily antiviral therapy. Condoms also reduce the risk of transmission.
