Herpes oral sex

Hello Doctor,

I am freaking out a bit. I am a woman who gave unprotected oral sex to my new boyfriend.  We have been together six months and we spoke about his condition with genital herpes.  He had his first outbreak over 15 years ago and has been on a suppressive therapy since then.  He is a nurse and told me I have very little chance of contracting herpes orally.  He has not had an outbreak in a while, and there was nothing visible, so I gave him a bj. (it is all we have done sexually).  

Immediately after swallowing when we were done my lips felt tingly and numb as well as a little sore. It felt that way all day into the next day and my cheeks feel a little numb too. It is now he third day and the little bit of lip swelling I had went down and the numbness is going away, although it did feel as if the numbness was moving from my cheeks to under my lip.

I've been tested in the past and have been negative for herpes. Is there a good chance this is it, or is my paranoia in overdrive? It is now the 3rd day.

topical steroids can trigger herpes to reoccur. they also aren't meant for the delicate mucosa of the lip.

Would a topical steroid cause more problems?

I hope this doesn't double post.

I've tried lip balm, and Vaseline to no avail. It has been 10 days and it won't go away. I guess I'll try a hydrocortisone cream, but since there is no itching I don't know if it will work. I guess I can only try it and see.

I wouldn't use a topical steroid on your lip area.

I've been using Burt's Bees and anything with a high SPF, I've tried Vaseline and nothing seems to get rid of it. Since this is the 10th day I'll keep trying it and maybe add a hydrocortisone cream. Although since there is no itchiness I don't know if that would help.

are you using a good lip balm on your lips?

if it lasts more than a week, you can follow up with your provider but odds are they'd just tell you to slather your lips with vaseline and not do anything at this point.

I have another question Grace, so far the irritation to my lips has continued unabated. Sometimes it is less obvious, sometimes more. There is no swelling, and no discoloration, my appetite has been fine and I feel fine.

Do you have any idea what this could be? I tried the dermatology forum, but it is closed.

nothing wrong with moving slow :)  If this is going to be a long term thing, you need to have something to do together other than sex so building up a relationship is a good thing!  I used to put in my dating ads that I needed someone who could make watching grass grow seem like a fun night :)  

your risk really is incredibly low overall of contracting hsv2 from him. ask him the testing questions I suggested and go from there :)

How long does it take for symptoms to manifest? I've read anything from 2 days to 30 days?  

It's good to know I have a smaller chance of contracting it. It puts me more at ease.

I move very slowly, hence only getting to a blow job in 6 months. He's great and I don't want my ignorance and fear to get in the way. I want to educate myself regarding safe sex and how to be supportive of him.

well if you two did nothing but avoid sex any time he had anything going on genitally, you'd be 90-92% likely each year NOT to contract hsv2 from him, regardless of the sex act. since he's on daily suppressive therapy with valtrex, your risk is cut in half!!  If you throw in condoms for sex too, you are down to a 2-3% risk of contracting hsv2 from him each year.  Overall your risk is fairly low without a lot of precautions.

your oral symptoms are more likely from irritation and chapping from performing oral ( as well as some anxiety thrown in ). Use a good lip balm and if you notice any obvious cold sores, be seen within 48 hours of their appearance for a lesion culture and typing.

Thank you so much. I believe he has hsv2, I guess this is something I need to discuss further with him.

The smart thing for me to do is get tested again, I had a test done last year, but it wouldn't hurt to get tested again. I was just driving myself mad imagining every little thing.  

I've done some reading on it, but a little bit of knowledge is a dangerous thing. I've got to learn more.

herpes will not cause immediate symptoms like this.

does your partner have hsv1 or hsv2 genitally? since he was diagnosed 15 years ago, knowing what testing he had done is a good idea to see if he needs additional testing done or not to confirm which type he has.

are you likely to contract hsv2 orally? no you are not. his being on suppressive therapy lowers your risk even more. you can also have protected oral sex which would be zero risk for you.
