fungal infection or herpes?

I had sex 13 days ago with a new partner and during intercourse the condom slid off.  Don't know how long it was off before we noticed.  Could have been a 5 seconds or 5 minutes, or longer.  I've never had a condom failure before and I always use them except with my long-term partner.  I have multiple partners (don't judge me, lol).

I was tested for all STDs in May and was positive for HSV-1 (had it 23 years no surprise), and negative for HSV-2.  This new partner says she was tested in August and was clean (but she doesn't remember if HSV was part of the screen).  She's a trauma nurse and gets routine testing through her work, but I'm inclined it's for HIV and hepatitis, not herpes.

Yesterday I started having some very mild aching in the sides of the buttocks, deep, not superficial, and some mild achiness in my lower back.  Also, occasional nerve twinges in my chest, thighs, calves, foot, buttocks, penis, scrotum, and perineal area.  Each twinge lasted a few seconds. Maybe one twinge every 30 minutes or so, always moving around. One place on my lower abdomen/pelvis above my left leg had a recurrent itch.  Skin looked perfect, no issues.  

All the nerve twinging, itching, and aching is gone today except for a very mild ache in the PC muscle area.  I do have anxiety issues and may have been over-focusing on normal body sensations yesterday as the first thing I thought of was "oh-my-god-herpes".  So the twinges/itches/aches could be non-related.

Last night I noticed I have very small red dots on the glans of the penis and a general redness around the tip and I feel some urethral itching.  No discharge.  The dots are not raised when flaccid, and are clearly visible, but with an erection, the skin is very, very slightly bumpy at the dots and the color blends in (no visible dots), they don't itch.  They are definitely in the skin layer and below.  No swelling of the glans.  It's been 18 hours since the dots/redness appeared with no change in appearance.

I'm inclined to think it's a fungal infection of some kind (the dots and redness).  But I didn't know if that could account for the nerve twinging I had yesterday.  I'm inclined to think not.  I think I may have picked up an HSV-2 infection (but my HSV-1 antibodies are keeping it nearly asymptomatic), but maybe I also have a fungal infection.  Or neither.  

I did go and get tested for herpes (Herpeselect) yesterday, to establish a baseline.  Should have results Friday.  Will retest again in 12 weeks if negative.

My HSV-1 cold sores on my lip have a clear tingling and hot feeling prodrome and the lesions appear very quickly, like in hours.  This is not happening on my glans.  I don't know how similar genital HSV-2 outbreak behaves relative to an HSV-1 oral outbreak (different nerves and tissues are involved). Especially for an initial infection (I thought the symptoms would be most severe).  I get an oral HSV-1 outbreak about once a year.

My question: could these kinds of dots/redness be pre-lesional for herpes?  Or is it more likely some other cause?

My first outbreak with HSV-1 lasted about three weeks and became a huge sore, lip swollen like I'd been in a fight, oozing and raw the entire time.  It even left a scar on my lip.  It was throbbing and hot and tingly.

Later sores weren't as bad or lasted as long, but mine do last a few days, like 4 or 5 before they begin to heal.

Yours is not lasting very long.  85% healed in two days?  For a recent infection?  Not sounding very herp-like to me.  Especially if you've had recurrent negative blood tests.  

A small sore around the mouth that behaves like you've described could be impetigo.  I'm not a doctor, so I cannot diagnose it.  But maybe you should have it looked at again and ask them about bacterial infections rather than herpes infections.

It throbbed after I intially squeezed it. I didn't even know I had something coming until I was drinking something and felt a tender spot on lip line. The area was a little red and it hurt when pressed.

I squeezed and within hours a bump formed on lip line that had a yellow dot. I squeezed and liquid came out and made the area red resembling a cold sore. I never had the sore on my actual lip though. It's on the skin above lip around lip line.

I've swabbed in the past but never had liquid. This one did but it was too late to swab. So I blood tested again. Negative for HSV1 and HSV2. Been this way since a year and a couple months removed from an exposure. This is the third time I have had this type of bump around my lip line. It acts different then a pimple.

How long does yours take to heal from your first tingle to when it heals. It's day 2 for me and mine is 85% gone already. Blood tests are neg. Doesn't make sense

Mine occurs on the lip, right on the edge of the lip line, but definitely on the lip itself.

Does your sore throb, tingle, or feel hot?

Have you had the fluid swabbed and looked at, or are you having blood tests run?  How long have you been having these sores?

I'm curious to your HSV1 oral infection. Does yours occur on your actual lip or lip line/slightly above your lip. Also, how long does it take to go away from start to finish.

I have what looks awfully similar to a cold sore on my lip line but my HSV1 test results are negative and always negative. Clear yellowish liquid comes out if squeezed and my lesion is very tender and hurts when pressed. It has to be a cold sore, but its healing quickly.

Your input would be appreciated.

Original poster here.  

It's been over 12 weeks since my initial dubious sexual encounter.  This week I got tested for all STDs and my results today show no HSV-2 infection.  IgG results less than 0.90.  No other infections, either, other than my very old HSV-1 infection.

I may get tested for herpes again in a month for the 16 week data point, but I'm pretty certain I don't have it.

Back to the original issue with the redness, spots, various tingles and zaps.  None of that has really changed.  Still there.  Sometimes have cloudy urine.

I also had a kidney stone soon after all this started.  Passed one stone, the other is still lodged in my kidney.  I think my twinges and zaps were very very small stones going through my bladder.  Just when a stone gets lodged against the ureter opening into the bladder you get all sorts of unpleasant penile and groin pain, FYI.  I think tiny stones are what are hitting that area and causing those sensations, not herpes.  The cloudy urine could also be caused by a stone, too.  Or, I could have prostatitis, but for purposes of this discussion, it's not an STD issue.

The penile redness and red spots are still a mystery, but not an STD at least.  And they don't actually bother me.  The tingles and zaps, though distracting, are not an issue, either.

Reading up on kidney stones, and how common they are, I think some people get very small stones that pass easily, but on their way out they cause odd groin and genital sensations.  Some of the people that post on this forum with symptoms similar to mine may be due to those tiny stones.  Food for thought.

Hello,

Im having the following issues a month after exposure:
- Scatica pain in leg (pretty bad)
- Small paper cut in anal area
- Burning rectal area
- Headaches here and there.

I'm also suffering from a fungal infection (so the doctor says) can it all be related?

Yes, the IgG for HSV-1 was >5.00 and <0.90 or HSV-2.  Definitely have an old oral HSV-1 infection.

If the test number was 5 or above it could be considered an old hsv1 and almost certainly oral. But unless a sore is swabbed we dont know if this is genital hsv1 or oral but oral would be the logical as is very common infection from childhood.

I forgot to mention, I also had oral prodrome start about a week after this encounter and a couple of days before my genital tingling stuff started.  I treated the oral area to prevent an outbreak.

Got the IgG results back today.  Positive for HSV-1, negative for HSV-2.  So my baseline is established.

No bumps, sores, rashes, red dots, etc.

Did get an odd sharp pain in my urethral opening last night, lasted about 10 seconds and went away.  Not a tingle, more of a sharp, needle like pain, kinda "zappy" feeling, if you know what I mean.

I'm kinda wondering if what I'm feeling is my immune system fighting off an HSV-1 genital infection.  The nerve sensations I get are so short in duration, makes me wonder.

Time will tell.  I'll update as anything changes, and when I get tested in February.

New infections could be on both sides but old infections would only be on one side.

Update.

Red dots and redness are almost gone today.  No urethral itching either.

Haven't used any topical antifungal cream or anything.  Maybe mild urethritis, I'm thinking, and my immune system resolved it.

But the odd twinges, itches, and deep hip pain are back.

I had some itching of my buttocks last night, same spot on both cheeks.  Not intense, but noticeable.  Skin looked fine.

With a herpes infection, isn't it only one side affected?  Or could the prodrome and outbreaks be symmetrical?

Thank you!  I will keep this thread up to date on things.

I'm thinking it's a long shot I have HSV-2 from a single encounter like that.  

The nerve twinges and such had me freaked, though, lol.

Hi, none of what you mention would appear your having an hsv2 genital outbreak. Herpes are topical open sores, some small and hard to see and some blister or razor type cuts. They are not under the skin bumps and also almost never appear in the penis head but more the shaft.
Do let us know your test results but dont be surprized if they come back negative for hsv2.
