herpes or ?

I have had unprotected sex with a partner in January and had protected sex with a CSW on Apr 25.  On May 25 after two nights of no sleep and heavy drinking, I noticed a red spot on the shaft of my penis.  When I noticed the red spot, I also was experiencing an outbreak of HSV 1 on my upper lip (I get two a year maybe).  Three days later I had Herpeselect tests that were HSV 1- 5.22, HSV 2- 1.52.  The HSV 2 was a surprise as I have never had symptoms in the genitals

The spot itself was very small 3mm in diameter, I noticed no blistering or pain.  About 5 days later, I felt some aching in my lymph areas of my groin for about a day.  Also, at this time, the spot began to scab over, but it was not a dark scab, more like peeling skin.  8 days later two MDs looked at it, an IM and Derm, who said it didn't look like herpes. The derm surmised it could even be some of the eczema I get on my scalp, pretty severely at times, but wasn't very reassuring.  In fact, when he heard of my HSV 2 result (which I ordered myself) he smiled and sometimes you can't connect the two.  

It took about two weeks to disappear, and then reappeared in the same exact spot.  This time, what appeared to be blisters, but very small, not even .5mm and maybe two or three, appeared.  It actually took a magnifying glass to notice them There was absolutely no paid or irritation.  This healed the same way as the first spot, but less time.
There was no scar any indication of lesion for about a week.  The only symptoms were a little tingling and eposodic sharp pains in the head of my penis.

Now exactly one month after noticing the first, I have another red spot, same exact place, this one about 4mm and definitely what appear to be blisters (again one must use a magnifier to be certain)  These blister don't seem to present fluid and it is difficult to see if they have ulcerated. They are now appearing to scab (3 days later), but the scab is very light and not hard.  Also, the entire area, including the red area is beginning a peeling appearance, like scabbing/healing, but peeling.  This is three times in exactly 30 days.

There was absolutely no pain experienced with any of these presentations, very unlike the HSV 1 sores I get on my mouth, which are very painful.  Also, my penis has many papules on the shaft and head, and in the area the red spot develops. If these were the papules, I would say they are enlarging in the area of the red spot. I will admit I experience some of the penile pain and tingling mentioned earlier even before the red spot appeared initially and I do have a tendency to experience symptoms after reading about them.

I notified my partner from several months ago, she had tests done, which were supposedly specific, and was told she had an combined reading of 18 or so and she was definitely infected with HSV 1 and 2.  She had it done at an urgent clinic and they say she is infected and should have symptoms, but they cannot even give her the brand names of the tests they perfomed or seperate numbers.  She states she has never had any symptoms and the doctors there told here that was impossible.

My questions, if this is HSV 2 infection:

1) If I was infected 4 months ago, would I be experiencing these types of symptoms so late.  Or would this type of presentation (3 outbreaks in one month) be mored indicative of a recent infection (ie from the CSW).

2) Also, is the area the spots occur the area where I was initially infected. or could the virus entered somewhere else and just prefer this spot (remember, I used a condom with the CSW and this spot was definitely covered).

3) If infected from 4 months ago, and having had no noticeable symptoms until now, could I have possibly infected my wife?  I have abstained from sex this month during the red spots appearances, except during the short time I had no visible signs.  She has never mentioned any problems.  I do get HSV 1 and I think she has it too (orally), but never gets lip sores.  We also have oral sex (me on her) quite often.  Is there chance she and I could acquire HSV 1 genitally?

4) What type of doctor do recommend I see to have a culture perfomed?  

Until this recent occurence, I questioned the validity of the HSV 2.  I had another test (Herpeselect 1/2) run and will get the result tomorrow.  I wanted to use a Biokit, but cannot find who carries these, short of calling every office in my large city.

Your partner didn't have a type specific herpes igg blood test so her results are pretty much meaningless in this situation. She'll need to get a type specific test done that gives separate results for hsv1 and hsv2.

It sounds like you are having to look pretty damn hard to see these spots. If you have to get out a magnifying glass to see them - that's far more effort than you should be putting into it.  

Your previous hsv1 oral infection gives you significant protection against contracting hsv1 genitally later on. It can happen but the risk is greatly decreased.

I'd continue to follow up on this with your dermatologist.
