penis bleeding during sex

My boyfriend was diagnosed several years ago with HPV. he took medication and the warts went away and everything was good. that is until the last 6 months or so he began to experience pain and tearing on his penis during sex. Not rough sex, in fact barely even any sex. as soon as penetration would start is when he feels the first sign of pain. the tearing is on the side of his shaft  and the same spot everytime. and it bleeds. He said that is the area where the warts were several years ago. Does anyone know if this is a reaccurance of the HPV or lasting effect from having had it. I also want to stress that the sex is not dry, it is not rough and is not long. we have also tried condoms and the pain still occurs.  we wait for a long time for him to heal thinking it would be good to go. but it happens still.  first sign of pain we stop but the damage already happend and he is in pain and bleeding. If anyone can shed some light on this and if there is any way we can "toughen up" his skin,  I would appreciate it. .
