administering vaccine

MY MEDICAL ASSISTANT RECENTLY WAS GIVING AN IM INJECTION. DURING THE INJECTION SHE LOST CONTROL OF THE CHILD AND THE NEEDLE CAME OUT BEFORE ALL VACCINE WAS GIVEN SHE THEN RESTUCK THE CHILD USING SAME NEEDLE AND SYRINGE.

I'M TRYING TO FIND WHERE IT IS WRITTEN NOT TO DO THIS, SO I CAN WRITE HER UP FOR IMPROPER ADMINISTRATION. THIS WAS DURING HER 90 DAY EVALUATION

I feel for the poor girl. How many of us can administer an IM injection and hold a child at the same time time. I am sure that some of the responsability has to lie with the parent who has been asked and shown how to position the child.There for the grace of God go I!
Perhaps this is an ideal opportunity for reflective practice. She can learn from this. We cannot physically restrain children. I don't think you will find the research. Many clinicians I believe would have done the same. If she had drawn uo a second vaccine how would she have gaged the amount to give . It is difficult to comment on the situation because we were not there but I am sure we can all look back on our twenty or thirty years of nursing and can remember dificult situations especially with young children. I think you you need to assess her as a whole and talk through it with her.
What was the outcome of the consultation? Did she have evidence based research to
provide a rationale for her actions.
