How about a Liver Disorder Tracker.
The Hep C Tracker doesn't cover most Liver disorder problems , or test results.

Many of the labs can be included by adding a  Complete metabolic Panel,
You would still need to add PT/INR , AFP
A place to chart US ,EDG, Colonscopy & Biopsy Results.
Area for complications & Treatments Such as:
Ascites : Sodium restrictions , Diuretics  Dosage, Paracentesis , TIPS
Varices : Betablockers , Banding , TIPS
Hepatic Encephalopathy: Lactulose  dosage, Rifaximin dosage
  
Symptoms : Increased  abdominal discomfort , Loss of appetite , Increased weight. , Weight loss, Nausea,Vomiting , Diarrhea , Increased confusion, Jaundice, Liver Breath , Itching ,Dark urine, Blood in stools,
Area for current CTP score
Area  for current MELD score

These would help a lot and are the most followed
