trackers modification

On the Migraine, Mood and Sleep trackers (possibly other tracker re: same area but I don't use them) On bottom of several sections like Symptoms or treatment used --Can a other section be added so a person can specifically write their medication(s), treatment(s) and symptom(s)?  Also in the Migraine Tracker could the same be done for the  triggers section( there are many more ),? In the migraine tracker a box check system for 'where the pain is located' and how the pain feels, changes, moves, etc.    
These would be very helpful to keep an eye on especially when working with your doctor.
