Rash progression

Does any one know what makes the rash progress through the four stages? I through first it was hormonal or chemical, but I question that because one part of my daughters hand is at stage two and the rest of her body is still at one and has been this way for over a month. If it were chemical, why wouldn't the whole body transition to the next stage together?

Hi! My daughter gets stage 3 on her left foot and left hand. The rest of her body stays at stage one. I don't quite understand it myself.
