Help me out regarding treatment of Klebsiella Oxytoca as UTI?

Hi..my mother was having mild fever, burning sensation(type of irritation,no rash)of skin on face, palms and feet, and little tiredness for past 15 days. So we went to a doctor and he gave out to urine culture report. And that report says, Klebsiella Oxytoca(K.Oxytoca) is isolated in the urine. It is a urinary tract infection. It also says that the colony count of the bacteris is 1,00,000 cfu/ml.  Hence, doctor prescribed some medicines like Uribid with nitrofurantoin in it, Prothiaden M, Montemac FX for allergy and Hifenac-P for fever and also Cital syrup for UTI. My mother continued the course for one week, now fever and tiredness have decreased, but the burning sensation continues. Is this mixture of tablets fine, or any other suggestion or recommendations for treating this urinary tract infection of K.Oxytoca.Help me this out>>

Hi , Gabapin (Gabapentin also called Neurontin) is a neurologic drug probably given for the tingling. But the Nitrofurantoin has tingling as a less common side affect. So it will probably go away when you stop the antibiotic. If it is working she can stay on it until it is finished then the tingling will go away and she can get off the Gabapin too. Gabapin causes sleepiness or tireness as well.  I would ask the doctor and/or pharmacist if the tingling will go away after she gets off the Nitrofurantoin and if so then if she can tolerate it and it is working well then you can either up to dose of neurontin or try to tolerate it until the antibiotic is finished. If not then switch. I would not go off anything until talking to the doctor. Depending on the dose of Gabapin, she may have to taper off of it as you cannot go off of it immediately. You will get rebound insomina. Maybe some other side effects of going off too quickly. Unless she is at only 300 mg.

Sounds good. The tingling sensation sounds like a reaction to one of the drugs. It sounds like parasthesis. It is a tingling in the usually hands and feet and can be due to a nerve like issue either caused by a drug or some other problem like diabetes or other nerve issue. So overall things some like she is over the main problem. I would ask the doctor about the tingling issue. Is she diabetic? She didn't have this before the drugs and/or infection right? It may go away after the drugs are out of her system.

Hi, No..she is not diabetic. But after having Gabapin NT, it caused dizziness yesterday night feeling sleepy. Shall I ask doctor to change the antibiotic nitrofurantoin and keep away this gabapin nt?

Hi, we are not doctors on these sites but I am a microbiologist and can try to give you some advice. It depends on how resistant the particular strain of Klebsiella oxytoca is. They treat based on that. Do you know if they did an antibiotic sensitivity test as well as the culture? The only antibiotic I can see here is the nitrofurantoin (Uribid with nitrofurantoin). That is not that strong of an antibiotic but can work in some cases depending on the bacteria. She should finish the course of antibiotics and if she still has urinary tract pain she can get try taking lots of cranberry juice if she is not diabetic. If she is diabetic she can't drink the juice. Is it possible to find out whether they did further testing on the urine? Does she have a fever? You can also retest and get another urine test and urineculture done and ask if they would do the sensitivity test. Of course if she gets a lot worse see the doctor right away, such as a very high fever >101 F.

Hi,firstly thanks for ur response.Coming to the point, she was having fever and now after having Hifenac-P, fever was overcome.Now, after taking Uribid(Nitrofurantoin) dose, still tingling sensation in palms,feet and face continues for some time and again feeling normal. this is repeating daily.Yes, they did sensitivity test too and suggested antibiotics that are sensitive are (in order) nitrofurantoin,ciprofloxacin,norfloxacin,cefoperazone,amikacin. again when we went to doctor, he gave hicobal injection and Gabapin NT along with uribid and montemac as dose.Now no fever,but tingling sensation continues.
