Inguinal lymphadenopathy after handjob

The STD section is not taking questions, and I do not think I have an STD as it is unlikely according to posts I have read. Here is my story:

Almost 5 weeks ago, I had an erotic massage with hand job. It did not touch the 'therapist', except her breasts at the end. About a week later, I started getting a very localised pain just above my penis, and was proding it, a hard 'cord', and the more I prodded, there more it hurt, and over a few days I got anxious. With anxiety, I stated getting frequent urination, no burning at all or another UTI, prostatitis, like symptoms. I did loose appetite, lost 2-3 KG in a few days. During these days, when playing football, all pelvic pain went away. It also come in waves, from the oringal localised pain to more generalised burnng pain. On self inspection, I was convinced I had a tender and swollen prostate. I got severe constitpation, thought this was enlarged prostate related, and bloated severely even with weight loss, with a green tongue. After 4 days, a urine dipstick showed only ketones that where high (weight loss I pressume). I had a red strip dpwn the line that conects my forskin to my urethal meatus and down the underside of my penis in this time too, at least I thought it was more red than usual, that crease like line down the underside. As I do have some anxiety (panic attack) problems before, I thought my mind is getting the better of me. As handjobs are very low risk for STD, I thought it is all anxiety related. When I did see a GP on about day 6, she though the same, diagnosed IBS for the presentation of gut and anxiety, gave me antibiotics 'just in case' as I was concered for urethritis related STD even though I did not have clear presentation.

After that some of it cleared, I took only half the antibiotic course and stopped as I was trying to convince my self I was anxious. However, I started to then develop (now about 10 days after 'exposure') some very mild discofort in tip of penis, and pain in groin area and deep groin area that would intensify sometimes, improves, come and go, and sometimes with other non-specific symptoms, including the bloating on and off, and so on. Weight had stabalised through this. I had gone back to the GP, and she gave me a new course antibiotic, no examination, still thought it is anxiety, and I took these (ciproflaxin 500mg one daily for 5 days; Doxycycline? 100mg twice daily for 55 days) which I completed.

In this time, I continues to have some pains in and around the groin, lower abdomen, and so on. All this time I had this vague discomfort on the end of my penis. I also did not have any labido in this time, was disgusted with myself for going to this place, and soul searching about all this (I am seeing a psychologist to deal with previous childhood abuse I think this relates too). About 19 days after exposure (just as I was finishing the antibiotic course), I noticed a small, very small red lesion on the tip of my penis, associated with that it that connects forskin to penis head (no idea what that is called). No a vescicle, not an ulcer. Got concerned, but now convinced I am anxious. Then 3 days after that, I developed tender bilateral lymphadenopathy which has persisted for about 10 days now, although improving. This pain/small lesion is still there. In this time, my labido is returning, I have masturbated a few times, and it is OK, not painful, but not pleasurable. Another dipstick was normal. I did get deep aching left testicle, and left side pain is generally worse. The pain comes and goes, from localised to deep burning, and into upper thights. It is always worse thinking of it and driving the car (not always sitting, but alwasy driving somehow). I also get the lower back pain when driving too.

I have had an HIV test at 5 weeks, negative. No other test, and am certain this can't be STD? The presentation of a lesion related to HSV is late, as it was 3 weeks after exposure, bacteria should have been cleared with antibiotics, and from what I can gather STD from habdjob is like not possible.

I am 34 previously no problems. I had a small vesicle once before on penis shaft a few days after a ******* from a woman with fever blister active (I also have a history of fever blisters and so on), no major symptoms, and never came back again (this was about 2 years ago).

Now I am married, and I have told my wife what I did, and that caused intense emotional distress we are dealing with. However, she thinks that if I go for tests, it means I am still not tellin gher the whole truth and we did more than just a handjob. The antibiotics, GP visit and HIV tests she does not know about, as I know I have been honest with her on all that I have said and she will not believe this if I do tests. We are not sexually active since this event, as we need to rebuild ourselves emotionally first we have decided. I also have bot told her about my 'tender groin' yet, as again, she will think I am not telling the full truth.

So, is it possible to provide me with further advice here? Can this history be consistent with an STD of any sort? Can it all be anxiety, or proding the nodes. I was prodding these nodes alot in this time from anxiety, and since they have become enlarged. Can this be gut related, IBS, and so on, as I do also get lower abdominal pains and stool is inconsistent in character, volume, I do bloat on some days, green tongue comes and goes, appetite comes and goes over days, and so on. Also, when pain in there, it seems to get worse if I take aspirin, and local anaesthetic/atibacterial does not seem to help either when 'discomfort there'. The lesion is the same, and is a little more uncomfortable after masturbation. I do not get any unrinary symptoms either

I am not sure if this is consistent with anything, and need some anonymous advice on how to proceed here and what this could be?

Actually, it is not totally irrelevant. I am sorry you went through a lot in your life. The explanation helps me understand whether you had an exposure or not. It doesn't really sound like you did. If so it is pretty unlikely. But you since your last blood test was 2 months after the exposure HSV IgG is you should be clear. Glad you are getting counseling.

As for the rash, I don't think that stress caused that. It may not be infectious. It may be dermatitis or allergy or something else. Do your rashes itch? I don't know if I asked that. Some of the anti fungal creams don't work on fungi they are resistant. You did test negative though.But if you want to try another antifungal Terbinafine (Lamisil) works better. Otherwise, if the rash doesn't go away you may see a dermatologist. It doesn't sound like a STD.

You said you were positive for ketones in the urine but didn't mention if you checked if you were diabetic. I would check into that if you don't know.

Not sure what caused the testicular burning or pain. That might have been due to injury. If you don't have it now then you are probably okay.

Yes, she sort of gripped my penis between her thighs sort of thing as she pulled her body over me, I did not expect that. To be honest, I went for the massage with a naked woman and not the 'pelvic release', at least thats what I consciously told myself and justified it to myself, but this 'therapist' was having none of that and, well, it was not hard for her to convince me to finish the massage as she is trained sort of deal. That part caught me off guard, the pulling her front over my front, so no, not certain what touched what in those few seconds. Nothing else came close to my penis touching any other part of her other than the hands. I feel sick thinking of it, paying for this sort of thing. I have some psychological issues from previous childhood abuse, and been working through it with psychologists in last few months. But the anxiety for STD, this desire to be 'punished', is so strong still. Anyway, this is now irrelevant to the post.

So 2 days short of 12 weeks post my potential exposure, I had another negative IgG for HSV2. The did the IgM incorrectly at first, which was also negative. I had another PCR on a swab from urethral meatus and this little rash next to it, also negative, but the potential of 'false negatives' still get me. I think I must have confidence now that at 12 weeks a negative IgG is good enough to be quite certain what ever is happening to my genitals is not herpes.

I did a MCS on the swab too, and nothing, no bacteria, no yeast/fungi, nothing, so not fungal either. Then what? I have been using a vaginal antifungal for 7 days now, and no real improvement? Can the rashes, constant sensitivity, groin pain and all that actually be a manifestation of anxiety? Really? I can understand pain etc, and even lymphadenopathy if I was always prodding there, but rashes can't be imaginary can they?

Anyway, in case others read this in the future, this is my story, and so far so good. My wife and I have moved through the difficult issues, and we will likely make love soon enough again, can I be mentally strong enough to be certain I can't give her herpes, we will see, if only this sensitivity would go away now then it would help.

If you really want to recheck wait two months and recheck then you will be really really clear.

Okay so your original post said that the "hand " job touched her breasts but not the vaginal area. That would be zero exposure for HSV2. So, you don't know for sure if it got rubbed down there. If it did you didn't have sex. Could you have gotten from that. Hard to say. like you said very low exposure. I don't think you were exposed. So, now how low was your HSV2 test taken from the time of exposure? If at 2 months or longer the IgG was taken you are clear.

Would I be correct in thinking the following:

- chances the 'therapist' had HSV2 is high (lets say 100%)
- 10 - 20% chance she could transmit the virus on any given day
- 1 in 100 chance to transmit the virus to me with the handjob and possible very brief genital to genital contact which I do not to know if it even happened
- 10% chance of a IgG test for HSV2 to be a false negative at almost 11 weeks post exposure

These are conservative, but maybe overall put me at low risk, yet there was some form of lymph node groin issue with back pain for a week or so.

I know this is irritating if this is anxiety, so apologies for the post in that sense, and thank you for your comments. It has helped alot!

Okay, so great that you are negative for HSV2, and Gonorrhea and Chlamydia. Did you also check for Syphilis? I know you can't get it this way but since you are checking for STD's and saw a sore like thing you might as well get it done. You may have been allergic to her oils or whatever she used. Also, maybe she had scabies or something like molluscum contagiosium. It is contagious.  Molluscum contagiosum is spread by direct contact with the lesion of an infected person or by contact with a contaminated object such as towels or piece of clothing.

So maybe you should see a dermatologist for this.

I was satisfied with this for a few days, and now I am back wharted in terms of anxiet with the return of this rash, so now I think the IgG HSV2 test was too early, and so on and so on, so again am I over anxious or do I have something to be anxious about and getting false negative results at this stage. Sorry, posted  by accident - I am also negative for syphilis. Scabies and molluscum I think are not too likely, also not a concern To me, it is HSV2 with false positives or super anxiety for HSV2 in which negative objective results are not even good enough to convince me.

Hi TylerSA, We are not doctors on these sites but I am a microbiologist and understand these issues. So, it is not likely to get anything from a hand job. Do you know if she washed her hands? From what I have read I doubt very much that you got anything from this route. Bacteria and viruses die off quickly outside the body and also lubricants and other products kill off viruses. So I don't think you have to worry. You can get tested privately if you are worried and just want to check. But I don't think you need to. You can go to a clinic and tell them you don't want anyone including your wife to know your are being tested. There are privacy rules. But honesty is always the best policy.

But it sounds like most of your symptoms are resulting from IBS and other things. Swollen lymph nodes in the groin area can be from any type of infection and the nodes have to be about 10 mm in size to be enlarged. If smaller then that I wouldn't worry about them.

You can also double check that you don't still have a urinary tract infection by getting a urine culture done. Not all urinary tract infections or UTI's are totally symptomatic. So you may not know you have the same symptoms as a classic UTI. It is better to get a culture than pop antibiotics. Also, if you don't finish the course of antibiotics you can make the bacteria resistant to that drug. So never stop the course in the middle unless you have bad side effects or get allergic. Always ask your doctor first.

You could have prostatitis. So you could see a urologist.

The high ketones on the urine stick may mean you are diabetic. You may want to check into whether you have acidosis or diabetes.

Stop kicking yourself over this incident. It wasn't that big of a deal to me. I am a woman. We are human. Just be up front with your wife. It probably wasn't the hand job she was mad at it was that you didn't tell the truth.

I'm sorry you had a hard time when you were young. That probably is causing a lot of your stress. It is good you are working on it. It will help both you and your wife if you build confidence and so forth.

I think a lot of your symptoms are from stress. But maybe a urologist would help.

So, I hope this helped to give you some ideas going forward.
