Frequent urination

I am a 40 year old woman with the problem of frequent urination. I do not have diabetes or UTI and my ultrasound scan is normal. I underwent a D&C procedure in December of 2007. My urologist thinks that I should see a psychiatrist. I have a bulky uterus. I was 200 lbs three years ago, now I am 125 lbs, (through diet and excercise).

My gynec told me that I have over-exercised,(walking 14 km a day).

I feel constant irritation and pressure in my bladder. My life is on hold because of this problem. Should I see a neurologist? I have lower back pain also.

Hi,
You have the classic symptoms of Interstitial Cystitis--  pain, pressure or discomfort in the bladder, urethra, or vagina, as well as frequency and/or urgency to void.  I assume your urine culture was negative and you don't have significant blood in the urine on a urinalysis.  I assume your uterine fibroids are not big to cause you to look 30 weeks pregnant.  You don't have poorly controled diabetes spilling sugar in your urine. I assume you don't have any strange neurologic symptoms like numbness tingling, pain or weakness in your extremities.  I assume you have no other major health problems, GYN or GI problems.  If all the above is true then you certainly could have IC.  The diagnosis is made by exclusion of the above problems plus having the classic symptoms.
Your history of weight loss is a great thing but has no effect on bladder function.  You don't need to see a psychiatrist. This is a real organic pain problem and is not in your head. Low back pain is very common and may be from strained back muscles, not a pinched nerve problem. Keep exercising, that is only a good thing.  Other problems that can cause your symptoms include overactive bladder, a bladder tumor or stone in your ureter-- all less likely with the above history.

I'm surprised your Urologist didn't evaluate you for IC.  Every Urologist and Gynecologist should be at least aware of IC and its basic presentation, even if they don't have expertise in treating it. You need to go to the Interstitial Cystitis Association website-- ichelp.org, and educate yourself about the disorder.  Secondly you need a referral to a Urologist/Urogynecologist that specializes in IC. There is a list on the website.  There is help for you, and effective treatments to reduce the symptoms.  You need to see the right provider who will take a personal interest in you.  It's often a difficult and frustrating disease to treat but there is help.   Good Luck.

Hi,
Thank you for your advice. Actually I do have tingling sensation in my arms and feet and hot/cold flashes.

This is why I wonder if I should see a Neurologist. I have a very 'bloated' feeling most of the time. Could my bowels cause the pressure I feel on my bladder?

My urologist told me that I do not have IC, when I asked him directly, although he did not test me specifically for it, except an URINE CULTURE, URINEANALYSIS, ABDOMINAL SCAN and URINE FLOW RATE TEST.

I am trying to train myself to pass urine only once in 3 hours, although this is very hard at times!

I wonder if there is someone who could help me in Bangalore, India.
