AVNRT Discharge Summary

Is there a minimum of information to be included in a Discharge Summary?  Mine simply states I was treated for AVNRT of the slow pathway and that it was a success.  I asked for more detail for future reference.  I am not familiar with all the medical terms so I asked for the "Operation Notes" which I was told were not kept for this procedure.  I was given absolutely nothing regarding who performed what procedure (maping vs ablation) - there was a fellow involved.  I was provided nothing about the location or duration of RFW application.  I was provided nothing on mapping - not even telemetry reports taken overnight.  I am really upset about this whole thing.  My heart is really a mess - it has never been this bad before. I have premature atrial beats all day now - really strong knocking stuff.  It is so upsetting it makes it hard to heal.

1)  can you please provide me the technichal name of the reports I need to ask for regarding all computer generated technical equipment?  There must be a report for recording all mapping, diagnosis and ablation procdures.
2) Can you please provide me the technical names of all mapping recordings (ie xrays) I might possibly need in the future so I can get copies?
3) For typical AVNRT of the slow pathway - is there an acceptable entry point (right or left).  For some strange reason I remember being asked if I cared if they entered the right or left and I had no idea what they mean't, only that one required me taking aspirin prior to the procedure.
4) For the past two mornings I have had atrial fibrillations when I wake up.  I have NEVER had them before.  Should I be afraid of a stroke.  I called my physician who simply said to try to catch it on my event monitor.

Is there anything else you think I should ask for ie types of catheters, types of ablation equip.

Thank you so much for any advice.  I am really scared by all this.

You should ask for the computer generated report on your procedure as well as the dicated report.  Some labs have only a computer generated report which should give all the information you are asking.  You should talk to the doctor who did the procedure and  if  you can't get any results ask for the director of the cath lab or the cath lab supervisor.
I suspect that the procedure was done without entering your left heart.

The physicians involved were a Fellow and The Professor of the school of medicine.  The procedure was performed in an independent facility.  I have requested all medical records from both (within 30 days of the procedure) and not gotten it.  Who would be in possession and responsible for those reports?  I assume it is the hospital, but perhaps the equipment is owned by the school?  If the procedure is performed in the hospital, I think it would be the hospitals report, right?
Is it normal to have asprin before and Heparin (q?t?)@1000U/hr during a normal AVNRT procedure?
