ICDs for LQTS

ICDs for LQTS

What is your opinion on ICDs for Long QT Syndrome patients. Would you only recommend them in patients who have had cardiac arrest or recurrent fainting or would you recommend one in anyone with LQTS? How effective do you think beta blocker therapy is in LQTS type 2? Do you consider T-wave abnormalites as an added risk factor for Torsades?

Clearly if someone with a prolonged QT has a cardiac arrest they should get an AICD.  Beta blockers can be considered in Types 1 and 2 but NOT type 3.  If the patient fails with beta blockers or does not tolerate them then they could be considered for an  AICD.
I do not know if T-wave abnormalities are an added risk factor.
