Concerned over creatinine clearance and urine residue.

Concerned over creatinine clearance and urine residue.

Hi - I'm new here.  Not even sure which forum to put this in....

Not even sure where to start.  I'm a 32 year old female with chronic lyme disease and severe anxiety (specifically health anxiety).

At the beginning of October 09, I started noticing a weird film on top of my urine every single time I urinated, regardless of concentration.  It's always there - sometimes it appears like whitish residue, somewhat oily, fatty, milky, but it's never my entire urine, just floating on the top.  Sometimes it looks like I emptied a capsule of white powdery medication, sprinkled it on top, mixed it around, and there it floats. I never have burning or discomfort when I'm urinating.  I do sometimes experience flank pain.

The only thing I can narrow this down to is when I started taking Klonopin regularly, but again I cannot find anything online regarding Klonopin producing a visible residue in urine such as this. I'm on .5 x 3 for GAD and panic disorder.

I had a CT scan (for an ovarian issue, ruptured cyst) back in the end of May 09.  That was totally normal, except for the cyst.  I had a renal u/s in November due to this odd film and flank pain and that was also normal.

Labs are normal.

Most recent labs show:

serum creatinine at 0.68
BUN 8

Urinalysis has been normal other than low specific gravity which I chalk up to my hydration status (I drink at least 100 ounces of water plus a cup of coffee per day).

I recently saw a Urologist who did a urinalysis in office.  He said the only thing he saw were some crystals upon micro examination.  He said "I bet this is what you are seeing in your urine", I disagree. He also said I had hyperoxaluria based on my 24 hr urine coll. (dictated on his recorder) which is also false based on my 24 hr urine collection.  He said my oxalates were high, but again I don't understand this because they fell well within range at 0.21 mmol/spec when the ref. range was 0.11-0.46.  Is he that incompetent or am I not understanding something?

My 24 hour urine collection has left me with more questions, no answers.  The creatinine clearance wasn't even addressed at the appointment and I didn't see it until I walked out of the office.  I returned and asked about it, however the nurse was clueless and thought the lower the better - I knew better.

My serum creatinine was 0.68.

Creatinine clearance (adjusted for body surface) was 67 ml/min - the normal range is 75-115.  From what I understand, this signals kidney disease or damage.  Is this not true?  After I walked back in and the nurse addressed the doctor, she told me that it was insignificant and since my serum creatinine was normal, all was good.

All other levels for the 24 hr urine collection were within normal range, however my urine output for 24 hours was 3450 ml, again - I drink a lot, but this does leave my urine dilute, which I don't believe affects the creatinine cl.

I've had electrolyte panels done recently which have been fine, as well as liver panels (due to lyme and my treatment).  The only major thing off here is the creatinine clearance.  No protein in urine, no bacteria, no blood, nothing else abnormal..

Honestly, I'm at my wits end and looking for either reassurance and confirmation that it's okey dokey or a point in the right direction.  When estimating my GFR based on my serum creatinine, it's 107, but that means nothing if the true gold standard is the creatinine clearance....  I'm seeing conflicting information online.

I appreciate any input!
