high protein, normal creatine and muscle loss

My husband is in the medical field and most likely knows what is going on with himself, but will not fill me in.  He will only tell me that he needs to get more exercise and eat better.  He has said this for 18 years now!  I feel there is something going on that he is not telling me like the small heart attack he had two years ago.  He just told me and he never went to the hospital so now he has some heart damage!

We have five children at home and I am on the brink of losing my father to these very same issues. Can you help me know what may be going on so I can research and know how to talk to my husband about going in and getting his heart and kidneys checked?

1994  My husband has been denied insurance since he was in his thirties because he was over weight (6' 1" 315 lbs.) and had protein in his urine, no diabetes.  He went in for further testing on his kidneys.  The doctor said his kidney function was good, but when the doctor suggested a kidney biopsy he stopped his appointments.  He just diagnosed himself with benign proteinuria.    

2007  Over eleven years later, he tried for insurance again, but this time he was still over weight, he now had a sugar level of 149, an A1C of 9.2.  His Creatinine was 1.19.  Cholesterol was 297 and his Trigycerides were 693.  His urine protein was 335.0 and his microalbumin was 262.  Blood pressure is great.  He refuses all medicine.

In the spring of 2009, he suddenly lost a lot of weight.  He got down in the 280's.  It was very noticibly muscle loss from his arms and legs.  When I brought this to his attention, he told me it was because he has not been getting exercise.  I haven't pushed it because it always ends up in a fight, but I guess I am over due for a fight because I love him.  

With his protein this out of whack and his creatintine normal what would this suggest?  Any ideas?  Also, is there a kidney doctor to talk to on this site?  Thank you for your insight.   My dad needs a heart bi-pass, but they cannot do it because his kidneys are only functioning at 17%.  He begins dialysis tomorrow!
