lithotripsy hematoma

i have a huge hematoma around my kidney after lithotripsy. surgery was three weeks ago and its unclear if it has gotten bigger. had sonograms, cts and xrays. my urologist is not responsive as to what to do. shouldn't i be referred to a specialist? my pain is excruciating and ended up in the er yesterday only to determine its larger in size and there were no signs of liquifaction compared tothe songram i had 4 days ago which reported no change in size but liquifaction was seen. what do i do ? who do i believe? how should this be treated?

Wow!!
I am amazed to how many people this has happened to!
Myself had Lithotripsy on my right kidney 25th March. Procedure started at 9 am back home by 1:30. By 4 o'clock in the ER with 103 fever and the worst pain I've ever had. Stayed 6 days in hospital first two days in intensive care. Have had 4 CT scans with little results in hematoma getting smaller! Finally after a month starting to feel little better and body is starting to reabsorb the hematoma. You have to be very careful and have a good Doctor to monitor that you don't bleed out again. Doctor said it could take up to 3-4 months to completely been gone. I myself will never do Lithotripsy again!!

Take care

I had emergency lithotripsy on Nov 1 on both kidneys with a stent in left kidney that was removed at the end of the procedure.  My regular doctor is partially retired and another doctor in the practice handled the emergency procedure and two weeks of follow up without my doctors knowledge.  Immediately after the procedure, I was in agony and very ill.  I spent longer than usual in recovery due to vitals which they attributed to normal pain.  After a few days at home in agony and running fever of 101, I called the doctor.  He just offered more pain meds.  Over the next two weeks, I continued to run fever up to 101 in evening, in terrible pain.  This doctor took an X-ray and checked urine and said he was satisfied The pain was from sediment that would pass.  He would not address the fever.  After second week we went back in and my mom went with me and we demanded CT scan (great insurance).  He said he didn't want to expose me to more radiation.  I was so ill laying there on his table, my 70 year old mom was getting riled up.  He did CT and let us briefly see what looked like a huge kidney covered in a big shadow.  He said yep, hematoma, no big deal. It'll go away and most people don't know they have them.   Could this cause the fever and illness, we asked.  He said no.  If your worried go see a primary care doctor.  He said he was happy with my progress....several days later, I am in the hospital with fever, chest pain and very weak, fatigued.  High blood pressure and heart rate was 113.  I was in hosp for 3 days and had contrast CT along with every work up and test.  Every thing came back normal except the hospital it's was. Shocked by how large my sub capsular hematoma was.  It covered about 75 % of my kidney which is enlarged.  I was told that my fever is caused by my body reabsorbing this hematoma.  From what I've read, when you have a large renal hematoma and you have distress or fever, you should be monitored to make sure it's not getting bigger because you could still have a bleed, which is not good. As of today, over three weeks later, I am still running fever, but it is getting lower, and I am only know getting enough strength to even walk around the house.  Lithotripsy is not what I expected.  I pray you will get the medical attention you need and that you begin to heal

Hi Bill,

How are you? Renal hematoma after shock wave lithotripsy is more frequent than what is believed.  In subcapsular and perirenal hematomas, ultrasound may show progressing hematoma diameters. Have this evaluated by your nephrologist with careful follow-up and physical resting. Lumbar pain may persist up to six months in some patients with significant hematoma after shockwave lithotripsy.(ref: http://www.urotoday.com/52/browse_categories/stone_disease/renal_hematoma_after_extracorporeal_shockwave_lithotripsy_in_a_series_of_324_consecutive_sessions_with_the_dolis_lithotripter_incidents_characteristrics_multifactorial_analysis_and_review__abstract.html)

Take care and do keep us posted.
