shoulder muscle inflammation

I have had severe neck, shoulder, and upper back pain for about 6yrs which comes and goes but each flare-up lasts longer and pain increases. This only occurs on left side and at times pain radiates down the back of my left arm.  MRI  and x-rays of shoulder and cervical spine show no problems.  At same time I have very foul smelling urine, been checked and no infections.  Our work did a medical screening a few months ago and my uric acid and sodium levels were LOW.  Other symptoms include dry mouth, and it feels like I have grit or something in my eye all the time. Again usually only in left eye.  Could any or all of these systems be related.  Only thing I have come up with from research is possible kidney problem.  Help I am in pain and getting no where with doctors.
