Enlarged liver / enlarged glands & kidney scar

I have been to the GP with a Sensation/pain/discomfert under my right rib and right chest pains that goes into my shoulder.

I was sent for a mammogram, chest xrays, liver & galbladder sonar.

Results was that my liver is enlarged to 180mm, scar on my kidney's and swollen glands in my chest wall.. My dr advised I should loose wait (I am +-20kg over wait). However, I have the following questions?

Liver
a) How large should the liver be?
b) Should I change my diet, cut out alchol / coffee etc.
c) Why the increase now all of a sudden I have waighed 95kg for the last 10 years?

Kidney
The scar on my kidney's is it normal, will it affect me in the future? How did I get it.

Swollen glands in my chest wall
My chest is very sore, when I lift up my arm it is sore, from time to time when I breath it is sore. How do I just live with it? What caused it?

this can be fatty liver so losing some weight and cutting out alcohol and exercising may solve the problem.  i am unsure what the chest wall glands are--?lymph nodes? I do not the kidney issue will pose a problem.
