swollen stomach area

My uncle had liver transplant years ago and galbladder was removed at that time. Now he has cancer and is being treated with antibody therapy. Lately he has huge stomach, NOT fluid. We have had 4 ultrasounds done to check ascites. It is not. Seems it is gas! He has very swollen ankles, all red and rough skin. Ankles go down if he keeps them elevated. We dont want to stop treatment with antibodies but are afraid of this huge stomach. He has scarring on bile duct, what could it be? what to do?
