End Stage Liver Disase

My mom had a liver transplant in 2004, due to hep C & B (non alcholic). She was doing pretty good for awhile but,  has recently become pretty sick. She still has Hep C & also B and they have attacked her new liver. She has all the symptoms she had before her liver transplant. Swelling, confusion, easily brused. Shes on tons of meds. including lactalose. Which she wouldn't take regurally & has since ended up in a nursing home. They have diagnosed her with end stage liver disease. But while she been in there she seems to be doing much better, IDK? Her meld score at last check was 11, but her symptoms are all over the place. I am just wondering if the meld score is a reliable way to determine how much longer she has. I live about 4 hours away, & am tring to go back home asap. I dont want to think that shes ok if shes really not. The doctors will not give us much info. or they dont know themselves?
Has anyone else had this experience? Thank you

after a transplant the MELD score does not really reflect the severity of recurrent disease.  is she a candidate for retransplantation?
