Incivek for transplant patients

I know Incivek hasn't be approved or even trialed for transplant patients.  Since not FDA approved, does that mean a doctor cannot legally prescribe it?  Does anything (code of ethics, law, AMA) absolutely prevent a doctor from prescribing it?  The prescribing information doesn't say don't use but just lists the interactions, in other words it seems vague about whether a doctor could prescribe it for someone on prograf.

i do not think there is any such contraindication or preclusion to using the medication as long as the physician explains to the patient all of the potential issues and some of the uncertainties with its use.  the fact is that it will be very complex to use the medication and there are a lot of risks due to the affcets on prograf and cyclsoporine metabolism
