Wild blood pressure swings

My wife is 7 months post liver transplant and her blood pressue can go from 220/100 to 88/58 and faints within 8 hours.  Transplant team wants no part of it and blood pressure meds are being perscribed by local Doc.  They are kind of saying live with it.  She fell hit her head pretty bad and I  took her to ER and they saw it go from 138 to 197 and various numbers in between over 90 minutes.  She is of course stressed so they gave her panic disorder meds.

Any ideas how what to presue?

maybe switching to a long acting hypertensive is the way to go so there is a continuous effect throughout the day.  is she diabetic?
