Can a CT scan after TB tretment accurately point lung cancer?

After 4 months of TB treatment, a recent CT scan report of my dad came with the following:

The word spelling of Para Phyma in 4 is not correct.  Actually it is not very legible.  It must be some other word.

1.Trachea and bronchi are normal.

2.Calcification of wall arch of aorta, vascular structures are normal.

3.Multiple small soft tissue density noted in trachea regions. Largest measuring 15 at 9.2 mm.

4.Evidence of soft tissue density noted adjacent to anterior pigmental bronchus of right upper lobe involving adjacent lung para phyma ? of anterior and posterior pigments of right upper lobe with para phyma ? stranding and adjacent plural thickening.

5.Mild to moderate enhancement noted on contrast administration of small areas of necrosis within.

6.Multiple small air spaces of varying sizes involving anterior segments of bilateral upper lobes. Rest of the lung para phyma is normal in attenuation.

7.No evidence of any plural effusion. Visualized sections of liver and spleen are normal. Adrenal glands are normal both sides. No evidence of calcification. No evidence of bone erosion.

CT imaging features are suggestive of
• Bronchogenic carcinoma involving anterior & posterior segments of right upper lobe.
• Pneumotitis involving anterior segments of both upper lobes. Advised bronchoscopic biopsy.

Question:
My dad is 74 years old.  He lives in Hyderabad, India.  He was a smoker.  He has arrived at his own conclusions.  He is refusing to go for a biopsy.  I am not sure how to convince him that getting the biopsy test done is important.

I see the word carcinoma.  But is this a foregone conclusion.  I can not even figure out how long he has.  Does the report indicate where my dad is in the disease progression.

Your insights are appreciated.  Thanks for your help.
