Can a patient who has tracheal ETT aspirate?  If yes, what would be some causes?

i don't see how someone with a tube in their breathing tube (trachea) can aspirate (vomit stuff up and breathe it into lungs) since the cuff around the end of the tube keeps stuff from going into the lungs.
